Endometriosis is a chronic condition in which tissue resembling the uterine lining grows outside the uterus, causing inflammation and sometimes scarring. Symptoms may include disabling period pain, persistent pelvic pain, pain with sex or bowel movements, and difficulty conceiving. Similar symptoms can arise from other conditions, so they deserve assessment rather than being dismissed as normal menstruation. The diagnosis and treatment plan should reflect the person's symptoms, clinical findings and reproductive priorities.
A gynaecology consultation can review examination and imaging findings and decide whether symptom treatment or further investigation is appropriate. Laparoscopy is useful in selected circumstances, but is not mandatory before every diagnosis or treatment decision. Before seeking another opinion abroad, send ultrasound or MRI files, earlier operation reports and a record of treatments and their effects. Ask the team to separate pain-control goals from fertility goals and explain alternatives, uncertainties and continuing care needs.

A symptom history should cover menstrual timing, pain outside periods, bowel or urinary symptoms and effects on daily life. Ultrasound can identify ovarian endometriomas and some deep disease; specialist ultrasound or MRI may help map suspected involvement. A normal ultrasound does not exclude endometriosis, particularly superficial disease. Referral may still be appropriate when symptoms persist.
Laparoscopy may be considered when further clarification or surgical treatment is needed. The decision takes account of imaging, previous treatment, preferences and procedural risks. Clinical assessment can support treatment before surgery; there is no requirement for every patient to undergo an operation first.
Options may include analgesia, hormonal treatment and selected surgery. Choice depends on symptoms, suitability, side effects and whether pregnancy is being sought. Hormonal suppression used for pain control is not a treatment to improve spontaneous conception while actively trying to become pregnant. Discuss fertility concerns early rather than assuming the same plan serves every goal.
Removing lesions or adhesions may help selected patients, but symptoms can persist or recur. Surgery for an ovarian endometrioma can affect ovarian reserve, so its potential benefits and harms need particular discussion when fertility matters. Deep disease involving the bowel, bladder or ureter may require a team with relevant specialist skills. Hysterectomy is not a guaranteed cure for endometriosis.
Persistent pain may need coordinated support, including pelvic physiotherapy or other pain-management care where appropriate. Record what has helped, adverse effects and how symptoms affect work, sleep and relationships. Agree what improvement would count as a useful response and when the plan should be reassessed.
For an overseas opinion, include prior surgical photographs or videos if available, pathology and current reproductive plans. Ask which tests can be reviewed remotely and whether repeating imaging will change care. Request a written explanation of the proposed scope of surgery, alternatives and follow-up responsibility. Clarify medicines, appointments and any fertility consultation in the estimate. Sudden severe pelvic pain, fainting or heavy bleeding needs urgent local assessment, as another condition may be responsible.
No. Some disease is not visible on ultrasound; symptoms and specialist assessment still matter.
No. Surgery is selected according to the diagnostic and treatment question, not required universally.
No. Symptoms, imaging, previous surgery, ovarian reserve and fertility plans inform the decision.
No. Pain and disease may recur, and ongoing care may remain necessary after an operation.
