Crohn’s disease is a chronic inflammatory bowel disease that can affect different parts of the digestive tract, often the small intestine or colon. It differs from ulcerative colitis, and the distinction matters when choosing investigations and treatment. Diarrhoea, abdominal pain, weight loss or problems around the anus require assessment; symptoms alone cannot confirm the diagnosis. Some people develop narrowing, fistulas or abscesses, while others have a different pattern of illness.
A specialist gastroenterology assessment considers where inflammation is present, its activity, complications and previous treatment response. The aim is sustained disease control and protection of health, not a guaranteed cure from one medicine, diet or operation. For another opinion abroad, send endoscopy and biopsy reports, original scan files, laboratory results and a dated treatment history. Agree what further tests could change the plan and how prescriptions, monitoring and urgent advice will remain available after returning home.

No single test establishes Crohn’s disease in every patient. Blood and stool tests, endoscopy with biopsies and selected imaging help distinguish inflammation from infection and other causes of symptoms. Small-bowel imaging may be needed when disease extends beyond the area reached by colonoscopy. Perianal pain or drainage needs assessment for fistulas or abscesses.
Symptoms can improve while inflammation remains. Follow-up may therefore combine clinical review with inflammatory markers, such as faecal calprotectin, and endoscopy or imaging when appropriate. The team should explain what evidence will define response and when reassessment is needed.
Medicines may include biologic therapies, other immune-modifying treatment or selected small molecules. Moderate-to-severe disease may justify early advanced therapy rather than requiring everyone to fail a fixed sequence first. The choice considers prior response, complications, infection risk, other illnesses and personal priorities. Corticosteroids may be used briefly for disease control; they are not a routine long-term maintenance treatment. Infection screening and vaccination review may be needed before particular medicines.
Surgery can address selected strictures, fistulas, abscesses or disease inadequately controlled by medicines. It does not cure Crohn’s disease, and a postoperative prevention and monitoring plan remains important. Suspected obstruction or infection needs prompt assessment rather than waiting for an overseas appointment.
Nutritional assessment considers weight, deficiencies, symptoms and, for children, growth. A dietitian can adapt advice to disease location and any narrowing or previous surgery. No single restrictive diet cures Crohn’s disease; avoid removing whole food groups without an agreed plan.
For a specialist review, list each medicine, dose, treatment dates, benefit and reason for stopping. Include previous operation reports and any stoma details. Ask how monitoring, infusion or injection access, prescriptions and advice during a flare will be coordinated locally. Request an itemised estimate for consultation, record review, tests and ongoing treatment. Sudden severe abdominal pain, persistent vomiting with swelling, major bleeding or marked dehydration warrants urgent local medical care.
No. They are different inflammatory bowel diseases with overlapping symptoms and distinct patterns of involvement and treatment needs.
Not always. The specialist may use objective tests alongside symptoms to assess control.
No. Surgery is considered for specific complications or treatment needs, not automatically after diagnosis.
Do not change medicines independently. Discuss the benefits and risks of any change and agree a monitoring plan with the treating team.
