Hip osteoarthritis, also called coxarthrosis, affects the ball-and-socket joint between the thigh bone and pelvis. Changes involve cartilage and other joint tissues and may cause groin pain, stiffness and reduced movement. Pain can extend into the thigh or knee, making the source less obvious. Difficulties putting on socks, getting into a car or walking comfortably deserve an assessment; they do not by themselves prove that an operation is necessary.
Care starts with understanding symptoms, examining hip movement and considering other causes of pain. Treatment may include tailored strengthening and mobility exercises, activity adjustments, walking aids and medicines chosen with attention to personal risks. Weight management can be discussed when relevant and acceptable to the patient. If appropriate nonsurgical care leaves substantial pain or disability, an orthopaedic consultation can explore hip replacement, its limitations and recovery requirements. The plan should reflect daily priorities and general health rather than an imaging label alone.

Coxarthrosis can make the hip less flexible, with pain during walking or after sitting. Some people develop discomfort at night. Previous injury or abnormal hip development can contribute, but there is no single cause. Groin or knee pain can also come from another condition, so assessment should identify the actual source.
Sudden severe pain, a hot or swollen hip, fever or feeling unwell needs urgent local assessment. Inability to walk or bear weight after an injury requires emergency care; do not postpone this while arranging treatment elsewhere.
The clinician asks about walking distance, dressing, sleep, previous injuries and medicines, then checks gait, hip movement and strength. Typical osteoarthritis can often be recognised clinically. An X-ray may clarify structural changes or help surgical planning. MRI, CT and blood tests are reserved for particular questions rather than included automatically in every assessment.
Bring existing images and reports, a medicine list and a short account of activities you want to regain. Ask whether back problems, tendon pain, inflammatory arthritis or another hip disorder could explain symptoms.
Exercise should match current ability and progress gradually. Strengthening, mobility work and suitable aerobic activity can be planned through rehabilitation. Walking supports or practical aids for dressing may reduce difficulty with daily tasks. Discuss weight support individually, without treating a target weight as the whole treatment plan.
Pain medicines require review of other conditions and interactions. A selected corticosteroid injection may give short-term relief, but it does not restore cartilage. Ask what benefit is realistic, what risks apply and how response will be reviewed.
Referral to orthopaedics is reasonable when symptoms substantially affect life and suitable nonsurgical options are ineffective or unsuitable. Replacement addresses damaged joint surfaces. Choice of approach and implant depends on individual circumstances; a small incision or robotic assistance does not guarantee a better result.
Discuss infection, blood clots, dislocation and other personal risks, alongside expected benefits. Rehabilitation, walking aids, home support and follow-up should be planned before surgery. Recovery varies, and replacement is not inevitable for everyone with coxarthrosis.
Yes. Hip pain can be felt in the knee, but knee symptoms still need an appropriate examination.
Not routinely. It is useful when the clinician needs information that examination and other investigations do not provide.
No. Adjust painful activities and agree manageable exercise rather than stopping movement completely.
No. Symptoms, function, previous treatment, health and personal preferences also guide that decision.
