Osteomyelitis is an infection involving bone. Germs may reach it through the bloodstream, an injury or operation, or spread from nearby infected tissue. A foot ulcer in someone with diabetes is one important setting. Infection can develop quickly or persist and recur, sometimes around fixation devices. These situations require different assessments and cannot be managed with one standard treatment package. Children also need care appropriate to their age.
Persistent focal bone pain, swelling, warmth, a draining wound or new difficulty using a limb needs prompt medical attention. Fever may occur but its absence does not exclude infection. Specialists combine examination with selected blood tests, imaging and microbiological samples to identify the site and likely organism. Treatment may include antibiotics, drainage or removal of infected tissue, alongside wound care and support for bone stability. Severe illness requires urgent local treatment; a consultation elsewhere should never delay emergency care or essential antibiotics.

Acute infection may present rapidly, while chronic osteomyelitis can cause ongoing pain, a wound that drains or repeated episodes. Bloodstream infection, infection after a fracture or surgery, and spread from a diabetic foot ulcer have different clinical implications. Vertebral infection affects the spine and needs its own assessment. A joint infection is a related but distinct problem.
Arrange prompt local assessment for painful, warm or swollen bone, a concerning wound or recurring symptoms. A child who refuses to use a limb also needs evaluation. Severe illness with confusion or breathing difficulty warrants emergency care. Back pain with new weakness or bladder or bowel dysfunction needs urgent emergency assessment, especially when infection is suspected.
The team reviews wounds, operations, implants, previous infections, circulation, diabetes and immune suppression. Blood tests and cultures may help; imaging is selected for the location and question. MRI is often useful for suspected spinal infection, whereas an early plain X-ray may not show the full problem.
A bone or deep tissue sample can identify organisms and guide therapy. It is not required in exactly the same way for every patient. Clinicians decide sampling and antibiotic timing together; treatment must not be delayed in sepsis or neurological compromise. Never stop prescribed antibiotics yourself to prepare for a sample.
Antibiotic selection, route and duration depend on the organism, site, response and personal risks. Intravenous treatment is not automatically necessary throughout the course. Surgery may drain an abscess or remove dead and infected tissue. Implant decisions, reconstruction and stabilisation require specialist planning through orthopaedics, often with infection specialists.
Diabetes-related foot infection also needs attention to wound care, pressure relief and circulation. Selected patients may receive antibiotics without bone surgery; others require urgent drainage or further procedures. Neither approach applies to every case.
Bring previous culture results, operative notes, images and antibiotic records, including allergies or adverse effects. Agree who reviews the wound, treatment response and medicine safety. Movement and rehabilitation should follow the team's advice about bone stability and loading. Recurrence is possible, so new drainage, increasing pain or systemic symptoms need review.
Yes. Fever is not present in every case, particularly with persistent infection.
No. The need depends on abscesses, damaged tissue, stability, implants and the response to treatment.
Not always. Specialists may need a deep tissue or bone sample that better represents the infection.
No. Obtain medical assessment; an unsuitable antibiotic can miss the cause and complicate diagnosis.
