Planning HIPEC starts with deciding whether abdominal surgery and heated chemotherapy offer a reasonable benefit for a particular cancer. HIPEC means hyperthermic intraperitoneal chemotherapy: medicine circulated inside the abdomen during an operation. It is often paired with cytoreductive surgery to remove visible disease from the peritoneum, the abdominal lining. Eligibility depends on the cancer's origin, distribution, previous treatment and the person's ability to tolerate a major procedure.

Questions to resolve before accepting HIPEC

The first decision is whether the cancer can be treated appropriately with cytoreductive surgery. Specialists review its origin, spread within and outside the abdomen, and the chance of removing visible disease. The added value of heated chemotherapy depends on the diagnosis and protocol. A colorectal trial did not show an overall survival advantage from adding its particular oxaliplatin HIPEC regimen to surgery; this does not answer every question about other cancers or regimens.

Documents and preoperative assessment

Provide pathology, CT or MRI images, previous operation notes and details of systemic treatment. Further imaging or diagnostic laparoscopy may clarify disease distribution. Assessment includes nutrition, organ function and fitness for anaesthesia. Ask how the proposed operation could affect bowel function and whether a temporary or permanent stoma is possible. The clinic supplies individual fasting and medication instructions.

Surgery and heated chemotherapy

Under general anaesthesia, the surgeon removes visible disease where feasible. The extent of surgery may include affected peritoneum or other tissues. If HIPEC proceeds, a heated chemotherapy solution circulates through the abdominal cavity and is then removed. Findings during surgery can change the plan. Drug choice and technique are individual decisions, rather than one standard package for all patients.

Hospital recovery and complications

The team monitors pain, hydration, bowel function, blood counts and kidney function. Nutrition support, mobilisation and clot prevention form part of recovery. Risks include bleeding, infection, blood clots, bowel leakage or obstruction, kidney injury and chemotherapy-related effects. A major complication may prolong admission or require further surgery. Recovery is gradual and cannot be promised within a fixed timetable.

Returning home and comparing costs

Obtain written discharge instructions, emergency contacts and a plan for oncology review. Fever, increasing abdominal pain, persistent vomiting or breathing difficulty require prompt assessment.
