Astrocytoma refers to several brain and spinal cord tumours with astrocytic features. Their behaviour varies, and the name alone does not determine treatment or prognosis. Modern diagnosis combines the appearance of tumour tissue with molecular findings. In adult-type diffuse gliomas, IDH status is central to classification; childhood tumours often have different biological features and require a paediatric specialist team. Tumour location, grade, symptoms and the person's overall health all influence care.
If you are considering assessment abroad, arrange review by an appropriate neuro-oncology team before committing to treatment or travel. Prepare imaging files, pathology and molecular reports, a treatment history and current medication details. Ask whether the diagnosis needs specialist review, what additional tests are justified and how the proposed plan compares with care available locally. Discuss recovery, rehabilitation and follow-up arrangements, as well as what the written estimate covers.

Adult-type diffuse astrocytoma with an IDH mutation can be classified as CNS WHO grade 2, 3 or 4. Grade describes tumour characteristics rather than a sequence of stages through which every patient passes. Grade 4 IDH-mutant astrocytoma is distinct from IDH-wild-type glioblastoma. Pilocytic astrocytoma belongs to a different, circumscribed tumour group; one treatment pathway cannot cover all these diagnoses.
Childhood gliomas differ biologically from common adult diffuse gliomas. Assessment must consider the child's age, tumour location and molecular features, alongside effects on vision, development and neurological function.
Seizures, worsening headaches, weakness, changes in vision or speech, and problems with balance or memory can prompt assessment. These symptoms have other possible causes. A first seizure, sudden severe headache or loss of consciousness needs emergency medical care rather than travel planning.
A neurological examination and MRI help assess the lesion. Imaging alone usually cannot establish the complete tumour diagnosis. Tissue obtained during surgery or biopsy is examined by a neuropathologist, with molecular testing as appropriate. The team weighs whether sampling is safe, particularly for tumours near critical structures.
Options may include surgery, radiotherapy, chemotherapy or observation in selected circumstances. Surgery aims to balance tumour removal with preservation of function. For some IDH-mutant grade 2 astrocytomas after surgery, targeted treatment may be considered when radiotherapy and chemotherapy can be deferred. This does not apply automatically to other grades or molecular types. A child's plan requires separate paediatric review.
When comparing oncology services, ask who will review the MRI and pathology together. Share original DICOM images, prior scans and operative reports; confirm translation and tissue-transfer requirements. Request a written explanation of treatment goals, alternatives, relevant risks and proposed costs. Agree where follow-up imaging, symptom management and rehabilitation will take place, and who will review results after returning home.
No. Urgency and the role of surgery depend on the diagnosis, symptoms, location and assessment of surgical risk.
No. MRI provides structural information; tissue-based molecular tests can supply information needed for classification and treatment selection.
No. Different tumour biology and developmental needs affect the plan, including the timing and potential long-term effects of treatment.
No. Ask for an individual discussion of likely benefits, uncertainties and follow-up needs rather than a promised outcome.
