Medulloblastoma is a malignant brain tumour usually arising in the cerebellar region. It occurs more often in children, although adults can also be affected. Surgery aims to remove as much tumour as safely possible and obtain tissue for confirmation and molecular testing. The tumour can obstruct cerebrospinal fluid flow, causing hydrocephalus, and may spread through these fluid pathways. Assessment therefore extends beyond the visible brain mass.
Resection is generally one stage of treatment rather than the entire programme. Further therapy depends on age, molecular group, remaining tumour and whether disease has spread. A specialist team balances removal against risks to coordination, speech, swallowing and other functions. Families may consider care abroad for experienced paediatric or adult neuro-oncology assessment, surgical expertise and integrated rehabilitation. Before travelling, clarify how urgent pressure problems will be managed, how pathology and staging will be completed, and where subsequent treatment will take place. The largest possible operation is not always the safest useful operation.

Brain MRI shows the tumour and possible fluid obstruction; spinal MRI helps assess spread. Tissue testing confirms the diagnosis and molecular group. Cerebrospinal fluid testing may contribute to staging, but lumbar puncture must be timed safely after pressure problems are addressed. Hydrocephalus can require a separate drainage procedure before or alongside tumour removal. The team considers urgency before arranging an international journey.
The neurosurgeon reviews the relationship to the brainstem, nerves and structures involved in balance, swallowing and movement. The goal is the greatest safe removal, preserving function when the tumour cannot be separated safely. Families should understand whether residual tissue is expected and why. Preoperative review includes neurological examination, anaesthesia assessment, medicines and communication or nutrition needs. Instructions on fasting and medication changes come from the treating team.
Surgery reaches the tumour through a skull opening under anaesthesia. Tissue is removed for laboratory examination and the wound is closed. Afterwards, neurological checks, postoperative imaging and assessment of fluid circulation guide care. Support may be needed for mobility, speech, swallowing and feeding. Hospital stay and recovery vary with the operation and complications.
Risks include bleeding, infection, fluid leakage, hydrocephalus and neurological injury. Children can develop posterior fossa syndrome, sometimes called cerebellar mutism, with reduced speech and changes in movement, behaviour or mood. Recovery can require coordinated rehabilitation, and difficulties may persist. Reduced alertness, repeated vomiting, rapidly worsening headache or new neurological symptoms require urgent assessment.
Medulloblastoma treatment commonly combines surgery with chemotherapy and, in many cases, radiotherapy. Age, spread, molecular findings and residual tumour influence the programme. Very young children may need strategies that delay or limit radiation exposure. A clear scan after surgery does not remove the need for a treatment discussion within oncology. Long-term care may assess learning, hearing, hormones and rehabilitation needs as well as recurrence.
Request an estimate covering surgery, fluid drainage if needed, monitoring, molecular tests, rehabilitation and subsequent therapy. Include caregiver accommodation and support. Agree a safe travel date, emergency contacts and transfer of records so that returning home does not create an unplanned treatment gap.
Usually not. Additional therapy is selected according to the overall risk assessment.
No. Adults also need a specialist plan.
No, but their course varies and rehabilitation may be needed.
