A cavernoma, also called a cerebral cavernous malformation, is a cluster of abnormal small blood vessels in the brain or spinal cord. It is a vascular malformation, not a cancer, and differs from an arterial aneurysm or an arteriovenous malformation. Blood moves slowly through its vascular spaces. Some cavernomas are incidental findings; others cause seizures, bleeding or neurological symptoms. The significance depends on the lesion’s location and the person’s clinical history.
A neurosurgical consultation can review whether the cavernoma explains the symptoms and whether monitoring, symptom treatment or an intervention is appropriate. MRI findings alone do not mean every lesion needs removal. For another opinion abroad, provide original scans from different dates, reports, seizure details and any previous bleeding or treatment records. Ask the specialist to explain the options, uncertainties and follow-up needs. New stroke-like symptoms, a sudden severe headache or a first seizure needs urgent local assessment rather than waiting for travel.

MRI is the main investigation for cavernoma and should include sequences sensitive to blood products. The review considers previous haemorrhage, lesion number and nearby structures, including associated venous anatomy. Catheter angiography is generally not needed for a typical cavernoma; it may be considered when another vascular diagnosis, such as an arteriovenous malformation, is suspected.
Seizures, weakness or sensory changes need assessment in their own right. A scan finding may be unrelated to some symptoms. Multiple lesions or a family history can prompt discussion of genetic counselling and appropriate testing; not every cavernoma is inherited.
Selected cavernomas are managed without immediate intervention. Monitoring combines symptom review with imaging according to the clinical situation; there is no universal scanning timetable for everyone. Antiseizure medicines may be recommended when seizures occur, with treatment reviewed by a neurologist. Symptom control does not remove the cavernoma or guarantee that bleeding cannot happen.
Agree what new symptoms should trigger urgent reassessment and where to seek help. Driving, work and activity advice should reflect seizures, neurological function and the relevant local rules rather than a general online promise.
Surgery may be discussed for selected symptomatic lesions, previous bleeding or cavernoma-related epilepsy. The balance differs for an accessible lesion and one in the brainstem or another sensitive area. Possible benefits must be weighed against risks of neurological injury, bleeding and incomplete symptom control. Surgery does not guarantee seizure freedom or recovery of an existing deficit.
Stereotactic radiosurgery has a limited, selected role and uncertainties; it is not a routine alternative for an asymptomatic cavernoma. A specialist should explain why any proposed intervention fits the particular lesion and how follow-up will be arranged.
Send original DICOM MRI files, dates of suspected bleeding, seizure descriptions, medication lists and prior operation reports. Ask whether the diagnosis or symptom relationship needs additional review before repeating tests. Clarify which team will manage seizures, rehabilitation if needed and imaging after returning home. Request a written comparison of observation and treatment, with practical recovery arrangements. The estimate should separate consultation, image review, additional investigations, intervention and follow-up.
No. They are different vascular abnormalities with different imaging and management pathways.
Not automatically. Its location, symptoms and bleeding history help determine whether observation is appropriate.
Medicines can treat symptoms such as seizures, but do not remove the vascular lesion.
No universal rule applies. Genetic counselling can guide family assessment when an inherited form is suspected.
