A cerebral aneurysm is a bulge in the wall of an artery supplying the brain. Many unruptured aneurysms are discovered incidentally and do not cause symptoms. Their management differs from emergency treatment after rupture. A sudden, extremely severe headache, collapse or new stroke-like symptoms requires immediate local emergency care; do not wait for a planned consultation or travel abroad.
For an unruptured aneurysm, a neurovascular specialist assessment weighs the estimated risk of rupture against the risks and expected benefits of treatment. Size alone does not settle the decision: location, shape, growth, symptoms, health and personal history also matter. Some aneurysms are monitored, while others warrant discussion of an intervention. Before seeking another opinion, send the original vessel images and previous reports. Ask what findings support the recommendation, which alternatives are reasonable and how follow-up and urgent advice will be available locally.

CT angiography or MR angiography can show an aneurysm and its anatomy. Catheter angiography may be needed for additional detail or treatment planning, but is not automatically required for every incidental finding. A suspected rupture needs emergency investigations to assess bleeding and identify its source; this is a different pathway from routine outpatient imaging.
Observation may be appropriate when preventive treatment risks outweigh the expected benefit. Monitoring is an active plan with agreed imaging and reassessment, not a guarantee that rupture cannot occur. Growth or new symptoms can change the balance. Discuss smoking cessation and blood pressure management with the treating clinician. Screening of relatives is considered in selected family or inherited-risk situations, rather than automatically for every family.
Microsurgical clipping closes the aneurysm neck through an operation. Endovascular treatment reaches it through blood vessels and may use coils or other selected devices. Flow diversion is suitable for certain aneurysms, not a universal replacement for clipping or coiling. The team considers anatomy, rupture status, procedural risk and the medication requirements of any device.
Potential complications include stroke, bleeding, vessel injury and treatment-related neurological problems. Further imaging and sometimes additional treatment may be needed. A ruptured aneurysm also requires care for the consequences of bleeding, which can include intensive monitoring and rehabilitation. No procedure guarantees full recovery or removes every future risk.
Provide original DICOM images from each relevant date so that growth and anatomy can be compared. Include previous bleeding, family history, blood pressure information, medication lists and any operative or device records. Ask whether further imaging will change the decision and how its risks will be explained.
Request a written comparison of monitoring and the proposed treatment, including follow-up responsibility. Confirm individual instructions about medicines, recovery, driving and travel; do not stop prescribed treatment independently. The estimate should separate consultation, angiography if needed, devices, hospital care and later imaging. After a haemorrhage, rehabilitation and support at home may need separate planning.
No. Selected unruptured aneurysms are monitored after specialist risk assessment. Suspected rupture is an emergency.
No. Shape, location, growth and the person's history also inform the decision.
No. The relative risks depend on the aneurysm, proposed technique and clinical situation.
No. A sudden, exceptionally severe headache requires immediate emergency assessment, even without a known aneurysm.
