A pituitary adenoma is usually a noncancerous tumour arising in the pituitary gland at the base of the brain. Its effects depend on hormone production, size and pressure on nearby structures. Some adenomas produce excess hormones; others do not, but may still impair normal pituitary function or affect vision. An incidental scan finding does not automatically require surgery, and a small lesion can still cause significant hormonal problems.
Assessment usually brings together endocrinology, pituitary imaging and, where needed, visual assessment or neurosurgical advice. The treatment plan should distinguish control of hormone excess, replacement of deficient hormones and relief of pressure. For a specialist opinion abroad, send original MRI files, hormone results with dates and reference ranges, visual field reports and previous treatment records. Ask which findings need confirmation and how ongoing prescriptions and follow-up will be coordinated before arranging travel or assuming an operation is necessary.

Dedicated pituitary MRI helps assess the lesion and its relationship to the optic pathways. Blood tests investigate hormone excess and deficiency; additional targeted testing may be needed. Raised prolactin does not always mean a prolactinoma: medicines, other illnesses or effects on the pituitary stalk can also contribute. The endocrinologist interprets the results together rather than using one number alone.
Growth hormone excess can cause acromegaly, while an ACTH-producing adenoma can cause Cushing’s disease through excess cortisol production by the adrenal glands. These conditions need their own diagnostic assessment. A nonfunctioning adenoma may still cause low hormone levels or visual problems.
Selected adenomas without concerning hormonal or pressure effects may be monitored. The plan depends on the clinical context, growth and proximity to important structures. Prolactinomas are commonly treated with dopamine agonist medicines; surgery is considered in selected circumstances rather than automatically for every tumour.
Other hormone-producing adenomas or lesions threatening vision may need assessment for pituitary surgery. A transsphenoidal approach through the nose is often possible, but the operation and extent of removal depend on anatomy and goals. Risks include hormone deficiency, fluid-balance problems, bleeding, infection or cerebrospinal fluid leakage. Selected residual or recurrent disease may require medicines, radiotherapy or further intervention; no single approach guarantees complete control.
Long-term checks may include hormone testing, imaging and visual assessment. Hormone replacement can remain necessary after treatment. Confirm written instructions for essential medicines and when to seek urgent advice; do not adjust treatment independently. A sudden severe headache with new visual loss, vomiting or reduced consciousness needs emergency local assessment because pituitary apoplexy is one possible cause.
For another opinion, organise results chronologically and include previous operation notes and pathology if available. Ask what decisions can be made from existing records and which tests require attendance. Request a written plan covering treatment aims, alternatives, medication access, postoperative endocrine care and responsibility for results after returning home. A quotation should distinguish consultation, tests, intervention and subsequent monitoring.
Usually no. Pituitary adenomas are generally noncancerous, but their hormonal and pressure effects can still require treatment.
No. Observation or medicines may be appropriate, depending on the tumour type and clinical findings.
No. Hormone excess or deficiency can occur without visual symptoms.
Not automatically. Imaging, hormone assessment and sometimes replacement treatment remain important after an operation.
