Comprehensive lung cancer assessment brings together imaging and laboratory investigations to establish what a suspicious lung finding means. If cancer is confirmed, the results help identify its type and extent and guide treatment discussions. Assessment may follow an abnormal chest image or symptoms that concern a clinician. Cough and breathlessness have many possible causes; neither establishes a cancer diagnosis. The investigation pathway depends on the position of the abnormality, the person’s health and what earlier tests have already shown.
An overseas consultation may help clarify an uncertain pathology report, consider a suitable sampling technique or review which molecular tests are needed for a confirmed tumour. Send the original scan images, reports and details of existing biopsy material before travelling. Ask whether previous samples can be reviewed rather than collected again, and arrange a discussion of the combined findings. Do not delay your first local assessment while arranging travel. Coughing up blood or worsening breathing needs prompt medical attention.

A lung cancer assessment needs to establish whether an abnormality is malignant, identify its type and determine its extent. CT shows the lung lesion and nearby structures. PET-CT may provide additional staging information when indicated, although an active area is not necessarily cancer. Brain MRI is used when the clinical team needs to investigate possible spread to that area.
Laboratory examination of sampled cells or tissue establishes the nature of the finding. For confirmed cancer, further tests may identify gene changes or proteins relevant to treatment selection. The appropriate panel depends on the tumour and clinical circumstances. A liquid biopsy looks for tumour DNA in blood. It is useful in selected situations but does not automatically replace tissue sampling; most people still need a tissue biopsy.
A flexible bronchoscope allows the doctor to inspect the airways and collect samples. EBUS combines bronchoscopy with ultrasound to guide a needle towards suitable lymph nodes. A lesion near the outside of the lung may instead be sampled through the skin with CT guidance. Surgical biopsy is sometimes considered when other approaches cannot provide suitable material. These are alternatives, not a checklist everyone must complete.
The specialist weighs the information expected against the risks of the chosen technique. Discuss bleeding, infection and pneumothorax, where air collects around the lung. Increasing breathlessness, chest pain or substantial coughing of blood after sampling requires urgent medical help.
Provide earlier images, your medication list and details of allergies and existing conditions. Ask about medicines affecting clotting without stopping them yourself. Follow the department’s instructions about eating and drinking for your procedure and anaesthetic. Arrange an escort after sedation. Discuss any planned flight after a lung biopsy with the treating team before booking the return journey.
Bronchoscopy often takes about twenty minutes, although additional sampling may take longer. Preparation and recovery mean the hospital visit lasts longer than the procedure. Pathology commonly takes one to two weeks and sometimes more; molecular testing may follow a separate schedule. Completing the assessment and discussing the results cannot be guaranteed within one day.
Check whether the estimate includes specialist review, imaging, sampling, anaesthesia, observation, pathology and any indicated molecular tests. Arrange a written summary and transfer of results to your usual doctor. The oncology section provides related information.
Usually not. Examination of cells or tissue is generally needed to identify the abnormality.
No. Tests are selected to answer the questions relevant to your diagnosis and treatment.
Sometimes. The laboratory checks whether enough suitable material remains for review and further tests.
