Liver radioembolization delivers tiny radioactive microspheres through a catheter into arteries supplying a tumour. It is also called selective internal radiation therapy, or SIRT, and commonly uses yttrium-90. The particles lodge in small vessels and deliver radiation to the treated area over time. This differs from chemotherapy-based embolization and from external radiotherapy. It may be considered for selected primary liver cancers or liver-dominant metastases. Suitability depends on liver function, tumour distribution, blood supply and the wider treatment plan.

Treatment abroad may provide access to specialist interventional radiology, nuclear medicine planning and a multidisciplinary second opinion. Assessment includes checking whether the particles can be delivered safely and whether their expected benefit fits the patient's situation. A referral does not guarantee that treatment will follow the mapping procedure. Before travelling, clarify the required visits, recovery support, temporary radiation precautions and follow-up imaging and blood tests after returning home.

Selection and the treatment goal

The team reviews tumour type, liver reserve, previous treatments, blood tests and imaging. Radioembolization may aim to control liver disease, relieve symptoms or, in selected cases, support a pathway toward surgery or transplantation. Disease outside the liver affects the decision. Compare the role of this treatment with surgery, ablation and medicines through an oncology assessment.

Mapping before treatment

A planning angiogram shows the arteries feeding the target. A catheter is guided from an artery in the wrist or groin using X-ray imaging. Some vessels may be protected or blocked when necessary. A test tracer and nuclear medicine scans assess distribution, including passage toward the lungs or other organs. These findings guide personalized dose planning and may show that treatment is unsuitable.

Tell the team about medicines, contrast reactions, kidney problems, infection and possible pregnancy or breastfeeding. Follow individual instructions for fasting and medication changes. Planning and treatment often require separate visits; the interval and any staged approach depend on the clinical programme.

Delivery and early recovery

At treatment, the specialist positions the catheter in the selected liver artery and delivers the prescribed microspheres. Local anaesthesia and sedation are commonly used; some patients need general anaesthesia. The catheter is removed, and staff monitor the access site, pain and general condition. Imaging may check where the particles have gone.

Fatigue, reduced appetite, nausea, abdominal discomfort or fever can occur. Discharge, lifting, driving and return travel depend on recovery and the team's instructions. Ask for written precautions about contact with children or pregnant people and information to carry when seeking medical help.

Risks and later assessment

Complications include bleeding or vessel injury, contrast reactions, infection and radiation damage to the liver. Particles reaching unintended tissues can cause stomach or bowel ulcers, gallbladder inflammation or lung injury. Liver dysfunction may appear later. Severe or increasing abdominal pain, persistent vomiting, fever, jaundice, swelling or breathlessness requires prompt assessment.

Blood tests and CT or MRI evaluate response and liver health. Effects develop gradually, and successful delivery does not guarantee tumour control. Additional treatment may be needed. Arrange local follow-up and provide both teams with the procedure and dose records.

Costs depend on mapping, nuclear medicine scans, dose planning, microspheres, treatment visits, observation and supportive care. Clarify what remains payable if mapping shows treatment cannot proceed.
