Assessment for epilepsy starts with understanding what happened, not with choosing the largest collection of scans. Seizures can involve shaking, a brief loss of awareness, unusual sensations or movements without collapse. Similar events can have other causes, and one seizure does not always mean epilepsy. A neurologist or epilepsy specialist combines the history, examination and selected investigations to decide whether events are epileptic and, where possible, identify their type and cause.
A specialist opinion abroad can be useful when the diagnosis remains uncertain, episodes continue despite treatment or previous recordings need expert review. Send original EEG recordings, brain images, medical reports and a diary of events before the visit. A witness account or safely recorded video may add valuable detail, but neither establishes the diagnosis alone. The resulting plan should explain the findings and how care will continue locally. Urgent assessment after a first suspected seizure must not be postponed until international travel is arranged.

The specialist asks about warning sensations, awareness, movements, event duration and recovery. Witnesses can describe things the person did not notice. Include sleep patterns, illnesses, medicines and earlier episodes. If video is available, it supports the clinical history rather than replacing it. The assessment may also consider cardiac or metabolic causes of seizure-like symptoms.
Electroencephalography records electrical activity through electrodes placed on the scalp. A routine EEG can support the clinical diagnosis and help classify epilepsy, but a normal recording between events does not rule it out. Results need interpretation alongside symptoms: an EEG is not a stand-alone yes-or-no test.
If uncertainty remains, the team may select a sleep EEG, a supervised sleep-deprived recording, ambulatory monitoring or video telemetry. Video-EEG links visible events with the electrical recording. Longer monitoring may improve the opportunity to record useful information, but capturing a particular event cannot be guaranteed. The method and duration depend on the clinical question.
Brain imaging can investigate structural causes; it does not replace the clinical assessment or EEG. Previous images may be worth specialist review before repeating a scan. MRI preparation requires the department to check safety information about implants and other relevant circumstances.
For EEG, arrive with clean, dry hair without styling products and follow the department’s instructions. Do not stop antiseizure medicines or restrict sleep on your own. If sleep deprivation is requested, discuss its risks and arrange the transport the team recommends. Routine EEG is usually painless and often takes about an hour; video telemetry may involve several days in hospital.
Recording time is different from reporting time. Specialist EEG analysis may take days or weeks, and further tests can be needed. Before travel, ask about the planned monitoring, accommodation, report availability and results consultation. Costs may include neurology review, EEG type and duration, imaging and inpatient observation when required.
Agree who will explain the diagnosis, review treatment and arrange follow-up at home. Related information about medication treatment for epilepsy does not replace an individual plan. A seizure lasting more than five minutes, or repeated seizures without recovery, needs emergency help rather than a planned overseas appointment.
No. A specialist assesses its cause and the risk of recurrence.
No. The recording must be considered with the clinical history.
No. The team explains what information the chosen recording may provide.
