Epilepsy is a brain disease involving a lasting tendency to have epileptic seizures. Seizures do not always involve shaking or loss of consciousness: some cause brief changes in awareness, sensation or behaviour. A first seizure needs medical assessment, but it does not automatically mean epilepsy. The diagnosis can sometimes be made after one unprovoked seizure when recurrence risk is high, or when a defined epilepsy syndrome is identified.
A specialist neurology assessment reviews the event, possible causes and appropriate investigations before choosing treatment. Medicines are selected for the seizure type, age, health and individual safety considerations; persistent seizures may justify assessment at an epilepsy centre. For another opinion, provide witness descriptions, any safely recorded video, EEG reports, original imaging and a dated medication history. Agree an emergency plan and arrangements for prescriptions and follow-up. Treatment can improve seizure control, but no particular medicine or procedure can guarantee freedom from seizures for everyone.

History and witness accounts help distinguish epileptic seizures from fainting, metabolic disturbances and other events. Cardiac assessment and blood tests may identify alternative causes. EEG supports diagnosis and classification, but a normal routine EEG does not exclude epilepsy. Additional recording, including video EEG, may answer an unresolved question. MRI can investigate structural causes when indicated; imaging findings also need specialist interpretation.
Antiseizure medicine selection depends on seizure type or syndrome, age, other medicines and potential adverse effects. Children may need syndrome-specific care and attention to learning and development. Pregnancy planning and contraception require early specialist review because risks and interactions differ between medicines. Do not stop treatment independently, including during pregnancy.
Record seizure frequency, missed doses and adverse effects. If seizures continue despite adequate trials of two appropriately chosen, tolerated treatment schedules, specialist assessment for drug-resistant epilepsy is important. This does not mean that all further medicines are ineffective. An epilepsy centre may reassess diagnosis and consider surgery, stimulation devices or a supervised dietary treatment in suitable patients. Referral does not automatically mean an operation.
During a convulsive seizure, protect the person from nearby hazards, cushion the head and time the event. Do not restrain them or put anything in their mouth. Once the convulsions stop, place them on their side and stay with them as they recover. Give rescue medicine only if it is prescribed in their plan and you are trained to use it.
Call local emergency services for a first seizure, a convulsive seizure lasting five minutes or longer, repeated seizures without recovery, breathing difficulty or serious injury. Follow an established individual emergency plan if it calls for help sooner. Urgent care must not wait for travel or a remote consultation.
Discuss individual precautions for water, heights, work and driving, together with seizure-related risks, including sudden unexpected death in epilepsy. Advice should fit the person’s seizure pattern and circumstances. For a second opinion, list each medicine, dose, duration, benefit and reason for stopping. Request a written plan covering monitoring, emergency contacts and local medication access. Clarify consultation, testing and continuing treatment costs separately.
No. Diagnosis depends on the circumstances, recurrence risk and evidence of an epilepsy syndrome.
No. A routine EEG can be normal in someone with epilepsy.
No. Specialist assessment considers diagnosis, treatment history, possible benefit and risks.
Only after an individual review and agreed withdrawal plan. Stopping independently can cause seizures to recur.
