Comprehensive assessment for multiple sclerosis combines a neurological history, examination and selected investigations. It is considered when symptoms or earlier findings suggest a disorder affecting the brain, spinal cord or optic nerves. Changes in vision, sensation, balance or movement have many possible causes, so an MRI finding or symptom alone is not a reliable diagnosis. A neurologist interprets the overall pattern and considers alternative explanations. Some people need follow-up before the evidence supports a clear conclusion.
An overseas opinion may help when a proposed diagnosis is uncertain, previous scan reports disagree or specialist review is needed before discussing long-term treatment. Send the actual MRI images, earlier reports and a timeline of symptoms, not just a diagnostic label. Ask which additional tests would be useful and how follow-up will be coordinated at home. New or worsening symptoms deserve local assessment without waiting for travel. Sudden weakness or loss of vision can indicate an emergency and should not be assumed to be MS.

The neurologist asks when symptoms began, how long they lasted and whether similar episodes occurred previously. Examination checks vision, eye movements, coordination, strength, balance and reflexes. This helps locate the problem within the nervous system and identify signs that may point to a different condition.
Diagnosis uses recognised clinical criteria and the combined evidence, not one positive test. The specialist looks for a pattern of central nervous system damage consistent with MS and evaluates other possible causes. When information is incomplete, further observation or repeat imaging may be more appropriate than an immediate diagnostic label.
MRI of the brain and, when indicated, the spinal cord shows areas of inflammation or damage. Earlier images are valuable for comparison. An abnormal scan does not automatically establish MS; the location and appearance of changes need specialist interpretation alongside the history and examination.
A lumbar puncture may be used to examine cerebrospinal fluid for supporting evidence. Under local anaesthetic, a needle in the lower back collects a small sample for laboratory analysis. Evoked potential testing measures how signals travel through selected nerve pathways using sensors on the skin. Blood tests and other targeted investigations help assess alternative explanations. Not everyone needs all these procedures.
Bring a symptom timeline, original scan images, reports and a medication list. Mention implants and difficulty with enclosed spaces before MRI. Before lumbar puncture, discuss medicines affecting clotting and relevant medical conditions; do not stop prescribed treatment without medical advice. Ask for the department’s specific preparation and aftercare instructions.
MRI duration depends on the areas and sequences being examined. A lumbar puncture typically takes fifteen to twenty minutes, sometimes longer, followed by observation. Fluid results may take days or weeks. The time spent on a procedure is therefore different from the time required to complete the diagnostic assessment.
Headache or back discomfort can occur after lumbar puncture. Severe or persistent headache, fever or increasing pain requires medical advice. Diagnostic uncertainty may require follow-up over a longer period; travel cannot guarantee a definitive answer during one visit.
Check charges for neurological review, MRI of each indicated area, fluid collection and analysis, evoked potentials and follow-up. Treatment decisions follow the confirmed diagnosis and disease assessment. The separate ocrelizumab information page describes one treatment used in selected patients, not a diagnostic test.
No single blood test establishes the diagnosis.
No. The findings need clinical interpretation and assessment of alternatives.
No. The neurologist decides whether it would add useful information.
