LOTCA, the Loewenstein Occupational Therapy Cognitive Assessment, is a standardised set of tasks used to describe cognitive abilities relevant to rehabilitation. Depending on the version, it explores areas such as orientation, visual and spatial perception, visuomotor organisation and thinking operations. An occupational therapist or another professional trained in the selected version observes performance and interprets the findings in context. The assessment can help identify strengths, difficulties and questions requiring further evaluation after conditions such as stroke or brain injury.
LOTCA is not an IQ test or a stand-alone diagnosis of dementia or another neurological condition. Before booking, clarify which version is proposed, why it suits the person's age and clinical needs, and the language of assessment. Vision, hearing, movement, fatigue and communication difficulties can influence performance. Ask whether the visit includes a written explanation and a discussion of practical rehabilitation goals, rather than relying on a score alone.

LOTCA provides a structured picture of selected cognitive skills. Its role is to support clinical reasoning about rehabilitation and identify areas that may need more detailed assessment. Results describe performance under particular assessment conditions; they do not establish the cause of a difficulty or determine someone's overall abilities.
The LOTCA family includes versions with different purposes and administration methods. A dynamic version examines the assistance a person needs during assessment. Confirm the full name of the proposed tool, the assessor's training and how its evidence and reference data fit the person's age, condition and language. Scores from different versions should not be treated as interchangeable.
Send relevant discharge summaries, previous cognitive assessments and the referral question in advance. Explain what is difficult at home, in education or at work, and what the person wants to achieve. Bring usual glasses and hearing aids. Tell the team about pain, tiredness, communication needs and difficulties using the hands.
Arrange language support before the visit if needed. Translation or changes to task delivery may affect how standard scores can be interpreted, so ask the assessor to record any adaptations. The team may plan breaks or separate sessions according to the person's needs.
A useful discussion connects the assessment profile with everyday functioning and the wider clinical history. Observations of daily activities, other assessments and information from the person or carers may be needed. Findings can inform individual goals within a rehabilitation programme, but do not promise a particular recovery.
Ask separately about time with the assessor, any additional sessions, the written report and the feedback consultation. These are different parts of the service. A quotation should specify the version, professional assessment, report language, interpretation support and any follow-up appointment. Further specialist assessment may require a separate booking.
No. It assesses selected cognitive and perceptual skills for clinical use. It cannot be converted into a general intelligence score or used to label a person's potential.
It cannot establish that diagnosis on its own. Concerns about cognitive decline require a clinical evaluation, with the choice of tests guided by the person's circumstances.
A clinician may recommend reassessment when it addresses a rehabilitation question. Interpretation should consider the version, interval, previous exposure to tasks and changes in health or assessment conditions.
