Ovarian cancer surgery establishes the extent of disease and removes cancer where this is feasible. For epithelial tumours, it may include the ovaries, fallopian tubes, uterus, omentum and other involved tissues. Advanced disease can require complex cytoreduction: an operation aiming to leave no visible tumour, balanced against the patient's ability to tolerate surgery. Some patients start with chemotherapy and have surgery after reassessment; a larger operation is not automatically the right first step.

Ovarian cancer includes different tumour types. Epithelial cancer often follows pathways shared with fallopian tube and primary peritoneal cancers; germ cell tumours require different decisions. A gynaecological oncologist assesses imaging, general health and the likelihood of removing visible disease within a multidisciplinary cancer treatment plan.
Primary cytoreduction may be appropriate when complete removal appears feasible with acceptable risk. Otherwise, chemotherapy followed by reassessment and interval surgery may be recommended. Tissue confirmation is needed before neoadjuvant treatment. Recurrent disease requires a separate assessment; repeat surgery does not suit every patient.
Surgery may remove both ovaries, tubes, uterus and omentum, with abdominal sampling to establish stage. Bowel or other involved tissues sometimes require resection, and a stoma may be necessary. Lymph node procedures depend on staging needs and suspicious findings; systematic removal of normal nodes is not automatically required in advanced disease.
For selected early tumours, preserving the uterus and an unaffected ovary may be possible after careful staging. This depends on histology, grade and extent, and cannot guarantee future fertility. Discuss reproductive wishes before treatment.
Preparation includes anaesthetic assessment, blood tests, nutrition, medicine review and planning for possible bowel surgery. Ask which procedures might become necessary during the operation. Follow the team's fasting and medication instructions.
Recovery involves pain control, gradual mobilisation, clot prevention and monitoring bowel function. Extensive surgery may require enhanced postoperative monitoring and nutritional support. Removal of both ovaries before natural menopause causes surgical menopause; symptom management is individual. Risks include bleeding, infection, blood clots, bowel leakage and urinary tract injury. Fever, increasing abdominal pain, persistent vomiting or breathlessness warrant prompt assessment.
Final pathology and stage guide chemotherapy; molecular findings and response help select maintenance treatment. Complete visible removal does not exclude microscopic disease.
