Radiation therapy for sarcoma is chosen according to the exact tumour type and location. Sarcomas are a varied group of cancers arising in soft tissues or bone, so one treatment recommendation cannot apply to all of them. For selected soft tissue sarcomas of the limbs or trunk, radiation is combined with surgery to reduce the risk of local recurrence. It may also be used when surgery is unsuitable or to relieve symptoms of advanced disease.
A specialist team reviews the biopsy, grade, size, imaging and expected surgical margins before deciding whether radiation is needed. When both surgery and radiation are appropriate, treatment before surgery is often preferred, but wound healing and later functional effects must be balanced. Planning aims to protect nearby nerves, joints, bone and organs. Rehabilitation and longer term follow-up are part of care, particularly when an arm or leg is treated. Patients may seek a sarcoma team abroad for expert pathology review and joint surgical and radiation planning, particularly for uncommon subtypes or difficult locations; ask how these decisions will be shared with clinicians at home.

Soft tissue sarcoma, Ewing sarcoma and osteosarcoma have different treatment pathways. Radiation has an important local control role in selected soft tissue sarcomas and Ewing sarcoma. For osteosarcoma, surgery is central to local treatment, with radiation reserved for particular circumstances. Specialist pathology review and staging should precede the decision. Radiation is not automatically required for a small, low risk soft tissue tumour removed with adequate margins.
For many limb or superficial trunk soft tissue sarcomas needing both treatments, preoperative radiation is preferred. It often treats a smaller area, but can increase wound healing problems. Postoperative radiation may be chosen when adverse features are discovered after removal or when the wound risk changes the balance. Its longer term effects can include stiffness and swelling. Routine radiation is not recommended for every retroperitoneal sarcoma; these tumours need separate assessment. Timing should be agreed by the surgeon and radiation oncologist.
CT planning is combined with relevant diagnostic images and information about the biopsy or surgical bed. The target and nearby organs are mapped for the chosen technique. Modern external beam methods shape the dose, but do not eliminate all risk. Brachytherapy or radiation during surgery is used in selected specialist settings. Prior radiation records are needed if areas overlap. For advanced disease, the goal may be symptom relief at a particular site. External beam treatment does not make the patient radioactive.
Skin irritation and fatigue are possible; other symptoms depend on the body area. Limb treatment may cause swelling, stiffness or weakness, while abdominal treatment may affect bowel function. Later effects can include fibrosis and weakened bone. A rehabilitation plan addresses movement and daily activities within surgical precautions. Report rapidly worsening swelling, severe pain, fever or wound problems promptly. Follow-up evaluates both cancer control and function. Read more about radiotherapy and oncology.
No. It can support a limb-sparing programme, but anatomy, tumour extent and the possibility of safe removal determine the final approach.
No. Its role varies by subtype and risk. Ewing sarcoma requires a systemic chemotherapy programme alongside local treatment.
No universal advantage is established. Suitability depends on the target, normal tissue exposure and the clinical evidence for the specific situation.
Not necessarily. Size change alone does not define response; the team assesses imaging and, when surgery follows, the removed tissue.
