Radioactive iodine treatment uses iodine-131 to deliver radiation to thyroid tissue that takes up iodine. In cancer care it may be considered after thyroid surgery to remove remaining thyroid tissue, reduce the risk of recurrence in selected patients, or treat iodine-sensitive disease that has spread. It mainly concerns differentiated cancers, including papillary and follicular cancer. The decision depends on pathology and recurrence risk; not everyone who has thyroid cancer needs this treatment.

The specialist assesses tumour type, surgical findings, spread and the likelihood of recurrence. Some lower-risk differentiated cancers can be managed without radioiodine. Medullary and anaplastic thyroid cancers do not take up iodine in the way needed for this treatment. If differentiated cancer stops responding to radioiodine, repeating it is not automatically useful; other treatment options may be considered within an oncology assessment.
