Immunotherapy is part of treatment for selected non-small-cell and small-cell lung cancers. Immune checkpoint inhibitors help immune cells act against the tumour by blocking specific inhibitory signals. They may be used alone, with chemotherapy or within a sequence involving surgery or chemoradiotherapy. The appropriate role depends on the exact cancer type and stage rather than the diagnosis of lung cancer alone.
Assessment reviews pathology, previous treatment, general health and relevant tumour tests. In non-small-cell cancer, PD-L1 expression and genetic changes can influence the choice between immunotherapy and other systemic treatments. These results need interpretation together; one marker is not a guarantee of benefit. Before arranging care abroad, establish a monitoring plan and access to urgent assessment for new symptoms, particularly breathing problems. Treatment requires ongoing review and coordination with clinicians who will supervise care after returning home.

Non-small-cell and small-cell cancers follow different treatment pathways. Within non-small-cell disease, pathology and molecular results further shape the plan. PD-L1 testing helps select some regimens, while alterations such as EGFR mutations or ALK rearrangements may favour a targeted treatment. High PD-L1 does not make genetic testing irrelevant or automatically establish the best option.
Selected resectable non-small-cell cancers may receive immunotherapy before or after surgery, sometimes with chemotherapy. Some locally advanced cancers have a role for checkpoint treatment after chemoradiotherapy. In metastatic disease, options can include a checkpoint inhibitor alone or a combination, according to eligibility and local approvals. These settings have distinct treatment goals.
For small-cell cancer, checkpoint inhibitors may accompany chemotherapy in extensive-stage disease. There are also approved uses after concurrent chemoradiotherapy for selected limited-stage disease without progression. The exact drug and sequence matter: a regimen for one category cannot simply be transferred to another.
Before treatment, the team assesses organ function, respiratory health, autoimmune conditions, transplant history and medicines. Clinical reviews, blood tests and imaging monitor safety and response. Duration depends on the regimen, disease course and tolerance. The plan should explain when benefit will be reviewed and how results are shared.
Immune-related lung inflammation, called pneumonitis, can cause new cough or breathlessness. Similar symptoms may arise from infection, cancer or other causes, so assessment is needed rather than assuming the explanation. Other effects include fatigue, rash, diarrhoea and inflammation of the bowel, liver or hormone-producing glands. Some reactions are serious or delayed; combined medicines have additional risks.
Take pathology, biomarker reports and previous scans to the consultation. Agree urgent contacts, tests between visits and follow-up at home. An estimate should specify the medicines, administration, imaging and care for complications included. General treatment context is available in the site's oncology materials.
Not necessarily. Some combinations have different selection rules from treatment with one medicine alone.
No. It acts on specific cancer features and can be preferable for particular molecular alterations.
Follow the team's urgent assessment instructions. New respiratory symptoms can have serious causes.
