Confirming the diagnosis and biomarkers

Non-small-cell and small-cell cancers follow different treatment pathways. Within non-small-cell disease, pathology and molecular results further shape the plan. PD-L1 testing helps select some regimens, while alterations such as EGFR mutations or ALK rearrangements may favour a targeted treatment. High PD-L1 does not make genetic testing irrelevant or automatically establish the best option.

Where immunotherapy fits

Selected resectable non-small-cell cancers may receive immunotherapy before or after surgery, sometimes with chemotherapy. Some locally advanced cancers have a role for checkpoint treatment after chemoradiotherapy. In metastatic disease, options can include a checkpoint inhibitor alone or a combination, according to eligibility and local approvals. These settings have distinct treatment goals.

For small-cell cancer, checkpoint inhibitors may accompany chemotherapy in extensive-stage disease. There are also approved uses after concurrent chemoradiotherapy for selected limited-stage disease without progression. The exact drug and sequence matter: a regimen for one category cannot simply be transferred to another.

Monitoring benefit and respiratory symptoms

Before treatment, the team assesses organ function, respiratory health, autoimmune conditions, transplant history and medicines. Clinical reviews, blood tests and imaging monitor safety and response. Duration depends on the regimen, disease course and tolerance. The plan should explain when benefit will be reviewed and how results are shared.

Immune-related lung inflammation, called pneumonitis, can cause new cough or breathlessness. Similar symptoms may arise from infection, cancer or other causes, so assessment is needed rather than assuming the explanation. Other effects include fatigue, rash, diarrhoea and inflammation of the bowel, liver or hormone-producing glands. Some reactions are serious or delayed; combined medicines have additional risks.

Planning care abroad
