The Wertheim-Meigs operation is a form of radical hysterectomy used for selected cervical cancers. It removes the uterus and cervix together with surrounding supporting tissues and part of the upper vagina, with assessment or removal of pelvic lymph nodes. The extent is adapted to the cancer and the treatment plan. Removing the uterus permanently prevents carrying a pregnancy; removal of the ovaries is a separate decision.

Diagnosis and staging establish whether surgery offers an appropriate balance of cancer control and complications. The team considers tumour size, depth, histology, imaging and nodes. Selected very early or low risk cancers may allow conisation, fertility preserving surgery or simple hysterectomy, under specific criteria. More advanced disease commonly requires chemoradiation.
