An HLA-B27 blood test identifies an inherited marker associated with certain inflammatory conditions affecting the joints and spine. A clinician may request it when evaluating suspected spondyloarthritis, including ankylosing spondylitis, alongside symptoms, examination and other investigations. A positive result does not mean that a person necessarily has the condition or will develop it later. A negative result does not exclude spondyloarthritis. The test contributes to a diagnostic assessment but does not determine treatment by itself or measure how active inflammation is.
A specialist opinion abroad may be useful when the diagnosis is uncertain, findings conflict or imaging and clinical evidence need review together. Travelling just to have blood collected is usually unnecessary; ask whether an existing laboratory result can be accepted. Send your symptom history, relevant family history, reports, images and medication list. Request an explanation of the reason for testing, the laboratory method and how the result will be discussed with a rheumatologist.

The test may support assessment of a suspected inflammatory condition involving the spine or joints. The pattern and duration of pain, morning stiffness, associated symptoms and family history matter. HLA-B27 is also present in people without an associated disease. It should therefore be selected for a clinical question rather than used to explain every type of pain. The clinician decides which other investigations are relevant.
Laboratories may detect the cell-surface antigen using flow cytometry or identify the associated DNA variant with a molecular method such as PCR. A positive result in someone with relevant symptoms can support suspicion, but cannot prove the diagnosis on its own. A negative result does not end the assessment when clinical concern remains. An equivocal result may require clarification by another method. Obtain the complete report, including the method and comments, rather than relying on a brief notification.
A venous blood sample is usually collected in the tube specified by the laboratory. Fasting is generally unnecessary for HLA-B27 alone, although other tests ordered at the same visit may have different instructions. Confirm preparation with the collection service. Tell staff about medicines affecting bleeding, previous fainting or difficulties having blood taken. Do not stop medicines yourself. Collection takes a few minutes; brief discomfort or a small bruise can occur. When a sample is sent elsewhere, the clinic must follow the receiving laboratory's requirements.
Collection time is separate from laboratory turnaround. The method, testing schedule, transport and further confirmation can mean waiting from several days to a few weeks. Ask for the expected date and how the report will be delivered. A rheumatologist interprets it alongside examination, inflammatory markers and appropriate imaging. Eye pain with redness, light sensitivity or blurred vision needs urgent eye assessment; do not wait for an HLA-B27 result or an overseas visit.
Check whether collection, analysis, sample transport, a confirmatory test and consultation are included. If testing has already been performed, send the report before booking; repeating it should have a clear reason. Confirm who will explain its implications and arrange follow-up.
No. Diagnosis depends on the combined clinical evidence, not a single marker.
No. Relevant symptoms can require further assessment regardless of the result.
No. The clinician uses other clinical findings and laboratory tests to assess activity.
