Bone marrow transplantation is a form of blood-forming stem cell transplantation. Healthy cells are infused into a vein to restore blood cell production after preparatory treatment. They may come from the patient or a donor, and can be collected from bone marrow, peripheral blood or donated cord blood. Transplantation is used for selected blood cancers, marrow disorders and immune conditions. The choice depends on the disease, previous treatment, organ function and whether suitable cells are available.
Patients may seek assessment abroad for a specialist transplant opinion, donor search expertise or a treatment pathway unavailable locally. Before travelling, confirm the transplant type, donor arrangements, expected monitoring near the centre and who will provide urgent care after returning home. The cell infusion is only one stage of a demanding treatment programme. Recovery and immune protection continue beyond hospital discharge, and neither a matching donor nor transplantation guarantees cure.

A transplant team considers the exact disease, its response to treatment, other options and the patient’s fitness. An autologous transplant returns previously collected cells from the patient, often after intensive cancer treatment. An allogeneic transplant uses donor cells and may add an immune effect against cancer. These approaches have different indications and risks; one is not automatically better.
Donor selection uses HLA tissue matching and other clinical factors, rather than blood group alone. Suitable donors may be relatives or unrelated volunteers; other cell sources can be considered by the team. Peripheral blood cells are collected by apheresis. Bone marrow collection uses needles, usually from the pelvic bones under anaesthesia.
Before conditioning, the team assesses the heart, lungs, liver, kidneys, infections and disease status. Discuss fertility preservation before treatment. Conditioning usually involves chemotherapy, sometimes with radiotherapy, to prepare for the graft. Its intensity depends on the transplant and clinical situation.
Cells enter the bloodstream through a venous line and travel to the marrow; they are not implanted surgically into the spine. Staff monitor reactions during infusion and check blood counts for engraftment afterwards. Infection precautions, nutritional support, medicines and blood product transfusions may be needed while counts are low.
Immune recovery continues after discharge. Follow-up checks graft function, infections, disease control and late effects. Donor recipients may require immunosuppression. Follow the team’s advice on food safety, vaccinations, activity and travel. Arrange local medical support and a direct emergency contact.
Risks include serious infection, bleeding, organ injury, infertility, graft failure and disease relapse. Donor transplantation can cause graft-versus-host disease, in which donor immune cells attack healthy tissues. Fever, new breathlessness, bleeding or significant diarrhoea need urgent contact with the transplant team. Costs depend on donor search, cell collection, conditioning, admission, complications and prolonged follow-up.
No. Bone marrow makes blood cells and is different from the spinal cord.
No. Autologous transplantation uses the patient’s own collected cells.
No fixed date applies. The team must confirm recovery and access to safe follow-up.
