Bone marrow cytogenetic testing examines chromosome changes in marrow cells to support the assessment of selected blood disorders. A haematologist may request it when investigating a suspected haematological cancer, refining an established diagnosis or reviewing treatment response. The laboratory question determines whether chromosome analysis, targeted fluorescence in situ hybridisation (FISH) or other tests are appropriate. This service concerns laboratory testing and interpretation; the appointment for collecting marrow is a separate part of the care pathway.
Before arranging testing abroad, send the referral, blood counts, previous marrow findings and any earlier genetic reports for review. Confirm which analyses are requested, whether a new sample is needed and whether the receiving laboratory can accept existing material. A normal finding cannot exclude every blood cancer, and an abnormal finding needs clinical interpretation. Ask for an itemised quotation, the expected reporting process and a consultation that explains how the findings affect the next steps.

Cytogenetic findings can help classify certain blood cancers and inform risk assessment or treatment planning. For example, investigation of suspected myelodysplastic neoplasms combines genetic results with blood counts and marrow examination. Acute myeloid leukaemia also requires an integrated assessment. The appropriate tests depend on the suspected disease and clinical purpose; there is no single marrow chromosome panel for everyone.
A conventional karyotype examines chromosome number and structure in dividing cells. FISH uses fluorescent probes to investigate selected chromosome regions or rearrangements. It addresses the targets requested rather than surveying every possible genetic change. Molecular tests, including sequencing or PCR, investigate additional genetic questions and are separate unless explicitly included in the agreed order.
Conventional chromosome analysis requires living cells for culture. The collecting team must follow the receiving laboratory's instructions for the sample, container, handling and transport. A formalin-fixed or paraffin-embedded biopsy block is not automatically suitable for this analysis. Some other assays may accept such material, but suitability must be confirmed for the particular test. Do not arrange shipment before laboratory acceptance.
Insufficient viable cells or unsuccessful culture may prevent a complete result. A negative study may miss small changes, low-level abnormal cell populations or abnormalities outside the method's scope. The report should explain what was analysed and relevant limitations. The haematologist interprets it alongside morphology, flow cytometry, other genetic tests and the clinical history.
For a second opinion, request the original laboratory report, methods, prior results and relevant pathology documents, with translations where needed. Discuss the findings within the wider oncology assessment. Ask separately about sample collection, transport, culture, additional probes, molecular testing and interpretation fees. Laboratory processing and the specialist consultation have different schedules; confirm how outstanding results will reach your doctor at home.
No. Puncture obtains the specimen; cytogenetics is a laboratory analysis performed on suitable material.
No. It describes the findings within that study's technical and sampling limits.
Not always. The haematologist and laboratory choose methods for the specific diagnostic question.
Ask the laboratory for its current estimate and whether culture, extra testing or repeat sampling could change it.
