Chronic lymphocytic leukemia (CLL) is a cancer of B lymphocytes that can involve blood, bone marrow and lymphatic tissues. It often develops slowly, and some people are diagnosed through a blood test before symptoms appear. Others experience enlarged nodes, fatigue, infections or other disease effects. A raised lymphocyte count alone does not confirm CLL or mean treatment must begin immediately. Related conditions, including monoclonal B-cell lymphocytosis, need to be distinguished.
Specialist assessment combines blood findings and cell-marker testing with symptoms, examination and relevant health information. Selected patients have active monitoring; treatment begins when clinical indications are met. If therapy is needed, choices depend on disease biology, organ function, other medicines and individual preferences. For a hematology and oncology consultation, prepare blood-count trends, flow-cytometry and genetic reports, previous treatment details and current prescriptions. Ask why monitoring or treatment is recommended and who will review results between appointments.

Blood counts and flow cytometry help identify and characterize a clonal B-cell population. The clinician distinguishes CLL from related lymphoid disorders and interprets the results in their clinical context. Bone marrow biopsy or imaging is selected for particular questions; neither is automatically necessary for every patient. Bring earlier results so the team can assess trends.
People without symptoms or concerning disease effects can often be monitored. This involves agreed reviews, examinations and blood tests. Progressive blood-count problems, significant symptoms or effects on organs may lead to treatment. The lymphocyte number alone is not the whole decision, and a risk marker does not automatically require immediate therapy.
Ask which changes should prompt contact before the next review. Monitoring can be emotionally difficult; discussing uncertainty and practical support is part of care. Do not independently start or stop treatment because of one laboratory result.
Before treatment, assessment of TP53 abnormalities, including deletion 17p, and IGHV status can guide choices. Some tests may need updating before a later treatment line. These tumour-related findings are not automatically the same as inherited-risk testing.
Targeted medicines play a major role in current care, sometimes combined with antibody treatment. Options include BTK- or BCL-2-directed approaches. Some plans have a defined duration; others continue according to benefit and tolerability. The choice considers molecular findings, previous therapy, kidney and cardiovascular health, bleeding risk, interactions and patient priorities.
Different medicines require different precautions. Some treatments need supervised dose introduction and monitoring for tumour lysis syndrome, a potentially serious metabolic complication. Ask about the schedule, blood checks and infection prevention rather than assuming an oral medicine needs little supervision.
Rapidly enlarging nodes or a marked change in general symptoms needs reassessment. A biopsy may be needed if Richter transformation into a more aggressive lymphoma is suspected. Symptoms or a scan alone do not prove this diagnosis.
Agree vaccination and infection advice with the team, along with a contact for fever or urgent concerns. For care abroad, clarify prescriptions, monitoring at home, result sharing and the estimate's inclusions. Keep a dated treatment summary and adverse-reaction record. Follow-up addresses disease activity and treatment effects.
No. Selected patients have active monitoring until treatment criteria are met.
No. Diagnosis and treatment decisions require the full assessment.
No. Duration and monitoring depend on the selected treatment; follow the agreed prescription.
No. It needs specialist assessment, sometimes with tissue sampling.
