Marginal zone lymphoma is a group of usually slow-growing B-cell non-Hodgkin lymphomas. The main forms are extranodal lymphoma of mucosa-associated lymphoid tissue (MALT), nodal lymphoma and splenic lymphoma. They affect different tissues and need different assessment and treatment pathways. MALT lymphoma can arise in the stomach or other organs; it is not the same disease as stomach adenocarcinoma. Symptoms depend on the site and may include enlarged nodes, abdominal fullness, fatigue or organ-specific changes.
A specialist review establishes the exact subtype and extent of disease using appropriate samples, blood tests and imaging. Some patients can have active monitoring; others need infection-directed treatment, radiotherapy or systemic medicines. The choice depends on symptoms, disease effects, location and health. For a consultation abroad, prepare the pathology report, available slides or blocks, original scan files, endoscopy reports if relevant and previous treatment details. Ask which findings need review and how care will continue locally. A general lymphoma package cannot replace an individual plan.

Extranodal MALT lymphoma arises outside lymph nodes, including in the stomach, salivary glands, lungs or tissues around the eye. Nodal marginal zone lymphoma primarily involves lymph nodes. Splenic lymphoma commonly involves the spleen, blood and marrow. A similar name does not mean that the same biopsy or treatment is appropriate for each form.
Diagnosis requires specialist interpretation of the relevant tissue, marrow or blood sample. Cell markers and selected molecular tests help distinguish related lymphomas. Staging and organ assessment guide planning; PET-CT and marrow investigations are chosen for the subtype and clinical question. A review through hematology and lymphoma services can clarify any uncertainty.
For selected gastric MALT pathways, Helicobacter pylori eradication is an important initial treatment. Testing, the exact regimen and confirmation of eradication belong to the treating team. Antibiotics are not a universal lymphoma treatment, and this approach must not be applied automatically to nodal or splenic disease.
Clearing the infection and assessing the lymphoma response are separate tasks. Endoscopy with biopsies may be needed after treatment, and a response can take time. Persistent findings require clinical interpretation rather than an immediate assumption that all treatment has failed. Gastroenterology and hematology teams should agree follow-up; other infections, including hepatitis C when relevant, may also affect the plan.
Active monitoring can be appropriate for selected people without troublesome symptoms or threatening disease effects. Localised disease may be considered for radiotherapy. Antibody treatment, chemotherapy combinations or selected targeted medicines may be used when systemic treatment is needed. Removing the spleen is not automatic for splenic lymphoma. Benefits, toxicity and alternatives should be discussed individually.
Rapidly enlarging nodes or a marked change in symptoms need reassessment; another biopsy may be considered if transformation is suspected. Before travel, send dated results, medicines and adverse reactions. Confirm whether pathology review is needed, request an itemised estimate and identify who will arrange results review and continuing care at home.
No. Extranodal, nodal and splenic forms have distinct clinical features and pathways.
No. Infection-directed treatment is used in defined circumstances, particularly gastric MALT; it is not a substitute for subtype-specific assessment.
No. Selected patients may have planned active monitoring, with clear instructions for new symptoms.
No. The team may need endoscopy and tissue assessment to evaluate the local response and plan surveillance.
