A bone marrow transplant is a treatment programme in which blood-forming stem cells establish new blood cell production after conditioning therapy. The term is often used alongside stem cell transplantation: the cells may be collected from marrow, circulating blood or donated cord blood. They are delivered into the bloodstream rather than surgically placed inside bones. Selected leukaemias, lymphomas, marrow failure disorders and other conditions may be treated this way, but the indication and transplant type require specialist assessment.

Planning involves more than finding a donor or booking an infusion. The transplant team reviews disease control, organ function, previous therapies and the patient's ability to tolerate treatment and prolonged monitoring. Assessing the transplant programme

An autologous programme uses the patient's collected cells to support recovery after intensive therapy. An allogeneic programme uses donor cells and can provide an immune effect against some cancers, alongside additional immune risks. Readiness and cell supply

Pre-transplant checks assess blood counts, disease status, infections and heart, lung, liver and kidney function. Discuss nutrition, physical activity, emotional support and fertility preservation before conditioning. Donor assessment includes HLA tissue compatibility and other clinical factors; a suitable family donor is not always available. Registry donors or alternative sources may be considered. Marrow collection from the pelvic bones under anaesthesia differs from peripheral blood collection by apheresis. The team confirms an appropriate cell product before treatment proceeds.

Conditioning, infusion and engraftment

Conditioning uses medicines, sometimes with radiotherapy, to prepare for transplantation. Its intensity is tailored to the disease and transplant strategy. Cells are then infused through a venous line, with monitoring for reactions. The waiting period until blood production recovers is called engraftment. During this vulnerable phase, patients may need infection prevention, transfusions, nutritional support and treatment of mouth or digestive symptoms. The infusion day does not mark the end of care.

Aftercare, complications and costs

Discharge depends on clinical stability and available support. Frequent follow-up continues because immune recovery takes longer than improvement in blood counts. Donor recipients may need medicines to prevent or treat graft-versus-host disease, where donor immune cells attack healthy tissues. Other risks include serious infection, bleeding, organ injury, infertility, graft failure and relapse. Contact the team urgently for fever, breathlessness, significant diarrhoea or bleeding. Agree vaccination, food safety, activity and travel guidance individually. Costs include cell procurement, conditioning, hospital care, complications and prolonged monitoring, plus accommodation and caregiver support.
