Confirming the diagnosis and goal

Biopsy establishes the lymphoma subtype; additional tests and suitable scans assess its extent. A haematology team discusses the plan. Aggressive disease may need prompt treatment. Active monitoring is appropriate for selected slow-growing lymphomas. Symptoms, risk and general health matter, not just the size of lymph nodes.

Selecting the drug programme

Hodgkin and non-Hodgkin programmes use different combinations. Some B-cell lymphomas receive chemotherapy with an antibody directed at tumour cells. Targeted medicines and immunotherapy have their own indications. If disease returns, another regimen, cellular therapy or transplantation may be considered. These are individual decisions, and a treatment used for one subtype is not automatically suitable for another.

Preparation and treatment cycles

Send pathology material, reports, imaging and previous drug records with dates. The team checks blood counts and organ function; some regimens require heart assessment or additional tests. Report infections, medicines and supplements. Discuss fertility, contraception and vaccination before treatment when relevant.

Some medicines are infused through venous access, while others are taken at home as prescribed. Cycles include recovery periods. Blood results and symptoms are reviewed before subsequent treatment, and a delay or dose change may be necessary. Outpatient treatment or admission depends on the regimen and clinical needs. Do not change the schedule yourself.

Side effects and safety

Fatigue, nausea, mouth soreness, bowel changes, hair loss or numbness can occur. Low blood counts increase infection or bleeding risk. Report an infusion reaction immediately to staff. Some regimens have late effects on organs or fertility, so discuss the specific risks rather than assuming all programmes have the same effects.

Obtain an emergency contact available outside office hours. Fever, shivering or sudden illness needs immediate contact, even without a high temperature. Breathing difficulty, severe chest pain or uncontrolled bleeding requires emergency care. Do not wait until the next cycle to report a serious problem.

Response, travel and costs

Examinations, laboratory tests and appropriate imaging assess response. PET-CT timing depends on the subtype and programme. After treatment, agree a follow-up plan. Clarify charges for medicines, administration, tests, support, admission and complication care. Transfer records and organise continuing care at home; travel should be agreed with the team.
