Brain SPECT is a nuclear medicine imaging technique that produces three-dimensional images after injection of a radioactive tracer. In perfusion studies, tracer distribution provides information about blood flow in different brain regions. A neurologist may request this examination in selected assessments of epilepsy or when the cause of cognitive symptoms remains uncertain after other investigations. It complements the clinical history, examination, MRI and, where relevant, EEG. A scan alone does not establish every neurological diagnosis or replace the wider assessment.

The scan shows patterns of tracer uptake related to regional brain blood flow. In selected epilepsy surgery assessments, images obtained after injection during a seizure are compared with images between seizures. This requires a specialist monitoring programme; it is not a routine outpatient appointment. In a dementia assessment, perfusion SPECT may help when clinical evaluation and structural imaging leave uncertainty. The neurology team decides whether the result is likely to change care.
Tell the department about possible pregnancy, breastfeeding, allergies, all medicines and difficulty lying still. Confirm instructions about food, caffeine and other substances that might affect the examination. Do not stop prescribed medication yourself, particularly antiseizure treatment. A resting perfusion study usually includes a quiet period around tracer injection. Discuss pain, anxiety, communication needs and any sedation arrangements before attending; the team must account for factors affecting blood flow and image quality.
A small amount of tracer is injected into a vein. After the required waiting period, you lie on a couch while a gamma camera moves around the head. It comes close but does not usually touch you. Keep still so that movement does not blur the images. Recording commonly takes approximately 30–60 minutes, with additional time for preparation and uptake. Some protocols add low-dose CT. Seizure-related studies may require separate scans and a longer admission. Confirm your own schedule rather than using the image acquisition time to plan a flight.
The examination involves ionising radiation. The referring team considers the expected benefit and individual risk. Follow the department's instructions about breastfeeding and close contact with children or pregnant people. Ask for confirmation of the examination if travelling soon afterwards, because airport detectors may detect residual radioactivity. A specialist reviews uptake patterns with previous scans, symptoms and EEG findings where applicable. Image abnormalities are not a complete diagnosis by themselves. Obtain the images, written report and a clear plan for clinical review.
