Resection of glioblastoma aims to remove as much tumour as safely possible while preserving important brain functions. Surgery can reduce tumour volume and pressure and provide tissue for a detailed diagnosis. Glioblastoma infiltrates surrounding brain tissue, so removing the visible mass does not mean that every tumour cell has been removed. Radiotherapy and drug treatment are commonly considered afterwards, with the plan adapted to the person's health and tumour findings.
The neurosurgical team assesses the tumour's position, symptoms, imaging and risks to movement, speech, vision and other functions. Sometimes a biopsy offers a safer way to obtain a diagnosis than a major resection. Patients may seek an assessment abroad for specialist surgical planning, mapping techniques or a second pathology opinion. A useful programme connects surgery with neuro-oncology, rehabilitation and care near home. Neither advanced equipment nor a larger resection guarantees cure, and decisions must balance tumour control with independence and quality of life.

A multidisciplinary team reviews MRI findings, neurological function and general fitness. Resection may relieve pressure and obtain tissue, but a tumour near essential brain pathways can limit how much can safely be removed. Biopsy may be preferable when resection would carry unacceptable harm. Discuss the expected benefit and the alternative plan within neurosurgery.
Provide scans in their original format, previous pathology, treatment records and a medication list. The team assesses anaesthetic risk, blood clotting and any existing weakness, speech difficulties or seizures. Additional functional testing may help plan surgery. Medicines for swelling or seizures are managed individually; do not change them without instructions. Arrange communication support and assistance after discharge.
A craniotomy creates an opening in the skull to reach the tumour. Image guidance and, when appropriate, brain mapping or monitoring help identify structures that must be protected. Some patients undergo an awake procedure for functional testing, while others have general anaesthesia throughout. The surgeon removes tissue within the agreed safety limits, then closes the skull opening and wound. These techniques reduce uncertainty but cannot eliminate neurological risk.
Afterwards, staff monitor consciousness, movement, speech and the wound. Imaging and tissue analysis inform the next stage. Molecular tests, including relevant IDH and MGMT assessment, help clarify the diagnosis and treatment discussion. For suitable patients, radiotherapy with temozolomide followed by further temozolomide is a common approach; frailty or other findings may require a different plan. Clinical trials have specific eligibility and uncertain benefits.
Rehabilitation may address mobility, communication or daily activities. Risks include bleeding, infection, swelling, seizures, fluid leakage and temporary or permanent neurological changes; serious complications can be life-threatening. New weakness, reduced alertness, a prolonged seizure or rapidly worsening headache needs urgent help. Wound leakage or fever requires prompt contact with the team.
Agree follow-up, wound care, medicine instructions and a safe travel date before leaving. Costs depend on surgical complexity, monitoring, hospital care, pathology, rehabilitation and subsequent cancer treatment. Returning home should not interrupt the planned transfer to neuro-oncology care.
No. Microscopic tumour cells can remain beyond the visible mass.
No. It depends on tumour location and the testing needed.
Sometimes, but benefit and risks require a new individual assessment.
