A prostate biopsy takes small tissue samples to check for cancer and describe its features if found. A urologist considers the need for biopsy using PSA results, examination, MRI findings and previous investigations. An elevated PSA or a suspicious scan does not by itself confirm cancer. Biopsy may also be used during active surveillance or when concern persists after an earlier negative sample.
When considering a clinic abroad, send the PSA history, MRI images and reports, previous pathology and details of medicines or urinary problems. Ask which biopsy route is proposed, how suspicious areas will be targeted and whether additional samples are needed. The assessment should also clarify anaesthesia, infection prevention and how the final pathology will be discussed. A tissue result helps plan care, but does not automatically mean that immediate treatment is necessary. Agree on follow-up and an itemised quotation before organising the visit.

Transperineal biopsy passes needles through the skin between the scrotum and anus; transrectal biopsy passes through the rectal wall. An ultrasound probe in the rectum may guide either approach. The transperineal route generally has a lower infection risk. MRI can help target suspicious areas, while other samples may examine additional parts of the gland. Ask the urology team to explain the proposed combination.
Report medicines, supplements, allergies, earlier biopsy complications and any urinary infection symptoms. A urine test or treatment of an infection may be needed before proceeding. Obtain specific advice about blood thinning medicines; do not stop them independently. Antibiotics, bowel preparation and fasting depend on the route, anaesthetic and local protocol.
Transperineal sampling can use local anaesthetic, with sedation or general anaesthesia in selected cases. Local injections may sting and the ultrasound probe can feel uncomfortable. Tell the practitioner if you are in pain. Sampling often takes about 20–45 minutes; preparation, checks on urination and recovery add time. Sedation or general anaesthesia requires agreed transport and driving restrictions.
A pathologist examines the cores. If cancer is present, the report may describe the Gleason score or Grade Group and how much sampled tissue is involved. This is interpreted with PSA, MRI and other findings; grade and disease stage are different. Results commonly take one to two weeks, but additional work can extend this. Arrange a discussion of the full report and next steps.
Soreness, bruising and blood in urine or semen can occur. Urinary retention and infection, including sepsis, are possible. Seek urgent help for inability to urinate, heavy bleeding with clots, fever, shivering or feeling seriously unwell. Follow the discharge instructions for activity, medicines and contact with the team.
The quotation should distinguish consultation, imaging review, biopsy guidance, anaesthesia, pathology and follow-up. Ask whether further scans or expert review carry separate charges.
No. The decision considers the overall clinical risk and other findings.
No. Persistent concern may need reassessment, monitoring or further sampling.
No. Selected lower risk cancers can be managed with active surveillance after specialist assessment.
Not automatically. Ask whether previous material and imaging can be reviewed first.
