Multiple myeloma is a cancer of plasma cells, immune cells normally involved in making antibodies. Abnormal plasma cells can build up in bone marrow and produce monoclonal proteins. The disease may affect bones, kidney function, blood-cell production and resistance to infection. Bone pain, unexplained fatigue or abnormal blood results need medical assessment, but none establishes myeloma alone. A monoclonal protein can also occur in conditions that require a different approach.
Specialists distinguish monoclonal gammopathy of undetermined significance (MGUS), smouldering myeloma and active myeloma using laboratory results, marrow findings and appropriate imaging. Monitoring or treatment depends on the exact diagnosis and risk; active disease can be defined by particular biomarkers even before clear organ damage. A hematology and oncology consultation should explain why treatment is recommended, how complications will be managed and which follow-up results are needed. Bring previous blood and urine tests, bone marrow reports, imaging and any treatment history.

Blood and urine tests can identify monoclonal proteins and free light chains. Bone marrow examination assesses plasma cells, while genetic studies may help classify risk. Imaging looks for bone or other disease. The specialist interprets these findings together with calcium, kidney function and blood counts; one abnormal protein result does not determine the treatment plan.
MGUS is not active myeloma and usually needs monitoring rather than myeloma treatment. Smouldering myeloma lacks myeloma-defining events, but its progression risk varies. Many patients are monitored; selected high-risk cases may be offered treatment according to current evidence and local authorization. Active myeloma may be established by organ effects or specific defining biomarkers, so waiting for symptoms is not always appropriate.
Drug combinations are selected according to disease risk, fitness, kidney function and previous treatment. They can include targeted medicines, antibody treatment and corticosteroids. The team should explain the goal, likely adverse effects, interactions and how response will be measured. Ongoing treatment or maintenance may follow an initial response.
High-dose therapy followed by return of the patient's own blood-forming stem cells is an established option for eligible patients. A transplant assessment considers suitability and timing; transplantation is not compulsory for everyone.
At relapse, previous drugs, resistance and the length of response guide further choices. CAR T-cell therapy and bispecific antibodies have roles in defined settings. Eligibility and local approvals matter. For CAR T-cell therapy, discuss cell collection. Both approaches require specialist monitoring and discussion of significant immune, neurological or infectious risks.
Care may include bone-protective medicines, pain control, infection prevention, treatment of anaemia and support for kidney problems. These measures are individualized, with monitoring for their own adverse effects. Discuss supplements and non-prescription medicines before taking them. New weakness, loss of sensation or bladder/bowel control changes requires emergency assessment, including possible spinal cord compression.
For a second opinion, send dated laboratory trends, marrow and genetic reports, scan files and a complete treatment summary. Request a written plan for result sharing, costs and care at home. Follow-up tracks both the myeloma and treatment effects; report new symptoms between appointments.
No. The result needs interpretation with marrow, imaging and other findings.
No. Risk assessment guides monitoring or selected treatment discussions.
No. Suitability depends on health, disease features and the agreed pathway.
Trends help the team assess change, response and when reassessment is needed.
