Primary cutaneous lymphomas are a group of non-Hodgkin lymphomas with an initial presentation in the skin. They differ from lymphoma arising elsewhere that also involves the skin. T-cell and B-cell forms have distinct features: some progress slowly, while others need prompt systemic treatment. Persistent patches, plaques or nodules, itching and changes in skin colour can prompt assessment, but many non-cancerous skin conditions cause similar symptoms. Appearance or a photograph alone cannot establish the diagnosis.
A specialist combines skin examination, biopsy findings and selected tests to define the subtype and extent. The assessment may include blood, lymph nodes and other organs; their relevance depends on the disease. Treatment ranges from observation or skin-directed measures to systemic medicines in defined situations. For a consultation abroad, prepare all biopsy reports, available tissue materials, dated photographs and details of previous skin treatments. Ask whether expert review or another sample is needed and how symptom control and continuing care will be arranged.

A skin biopsy is examined for cell features and relevant markers, sometimes with additional molecular tests. Early mycosis fungoides can resemble eczema and may require repeat biopsies over time. The dermatologist and pathologist interpret the sample together with the clinical history; an uncertain first result should not be treated as a definite diagnosis.
Staging checks the skin and, as appropriate, blood, lymph nodes or other sites. This helps distinguish a primary skin presentation from systemic lymphoma with skin involvement. Blood assessment is particularly important in certain T-cell disorders, including Sézary syndrome. Ask which findings establish the classification and what additional tests are necessary.
Mycosis fungoides and other T-cell conditions do not share one treatment plan with cutaneous B-cell lymphoma. Among B-cell forms, primary cutaneous follicle centre lymphoma often behaves differently from diffuse large B-cell lymphoma, leg type. The latter can be more aggressive, and its name does not mean lesions occur only on the legs.
Selected limited or indolent disease may be managed with observation, prescribed skin medicines, medical phototherapy, local radiotherapy or excision. The appropriate options depend on the subtype and clinical findings. Aggressive disease or particular blood or organ involvement may require systemic treatment. Antibodies, other targeted medicines and chemotherapy have roles in defined settings; there is no universal chemotherapy course for every skin lymphoma.
Care may involve dermatology and hematology, with radiation oncology and other specialists when needed. Discuss itching, pain, wound care, sleep and infection concerns alongside disease treatment. Report fever, rapidly worsening lesions or signs of infection promptly. Ask for clear instructions about prescribed creams and other medicines rather than changing them independently.
Send previous pathology reports and requested slides or blocks, not only the latest photograph. Include the duration and distribution of skin changes, treatments tried and their effects. Confirm a suitable method for sharing photographs and whether translations are needed. Ask what the written estimate includes and whether further sampling will be decided after examination.
Agree who will monitor skin changes, review blood or imaging results and manage treatment effects at home. Recurrence can occur, so a reassuring response does not automatically end follow-up. Keep a treatment summary and the specialist's contact instructions.
No. Many skin conditions look similar. Clinical assessment and appropriate tissue examination clarify the cause.
No. Their behaviour differs substantially; the exact subtype guides urgency and treatment.
No. The term describes the disease presentation. Extent and any later changes need specialist assessment.
No. Selected conditions can be managed with skin-directed care or monitoring; others require systemic treatment.
