Hormone therapy, also called endocrine therapy, treats breast cancers whose cells use hormones to grow. Eligibility is based on testing tumour tissue for oestrogen and progesterone receptors, known as ER and PR. The treatment reduces hormone production or interferes with hormone signalling in cancer cells. It may lower recurrence risk after surgery, be used before surgery in selected circumstances, or help control recurrent or metastatic disease.
The choice of medicine depends on the treatment goal, menopausal status, previous therapies and other health conditions. This is different from hormone replacement therapy for menopausal symptoms. Endocrine therapy is used for hormone receptor positive breast cancer. ER and PR testing helps establish whether a tumour is hormone sensitive; receptor negative tumours do not benefit from this approach. HER2 status, disease stage and other findings influence the wider treatment plan. Men with hormone sensitive breast cancer may also receive endocrine treatment.
After surgery, treatment aims to reduce recurrence risk. Before surgery, it may help shrink selected tumours. For recurrent or metastatic disease, the goal is control, often as part of a combination with other medicines. Options include drugs that block or affect oestrogen receptors, aromatase inhibitors that reduce oestrogen production, and suppression of ovarian function when appropriate. The same drug is not suitable in every hormonal situation.
Prepare pathology and biomarker reports, previous treatment records and a complete medicine list. The oncologist reviews menopausal status, fertility priorities, clotting history and bone health as relevant to the proposed drug. Some medicines are taken as tablets, others as injections. Ovarian suppression or removal requires a separate discussion. The duration and monitoring schedule depend on the clinical setting; there is no single course for all patients.
Hot flushes, fatigue, vaginal dryness, sexual symptoms and mood changes can affect quality of life. Aromatase inhibitors may cause joint discomfort and bone loss. Tamoxifen can increase the risk of blood clots and changes in the uterine lining. Risks differ by drug and personal history.
Report troublesome effects to the team so support or a treatment adjustment can be considered. Sudden breathlessness, chest pain or a swollen painful leg require urgent assessment. Unusual vaginal bleeding should be reviewed. Discuss all medicines and supplements because interactions can matter.
