Surgery is a main treatment for melanoma of the skin, particularly when disease is localised. After the initial biopsy confirms melanoma, a wider excision may remove additional skin around the site to reduce the risk of local recurrence. The planned margin depends on the tumour's thickness, stage and anatomical location. Some patients also discuss sentinel lymph node biopsy, which helps determine whether melanoma has reached the first draining lymph nodes.
A treatment plan combines pathology with examination and any staging tests needed. Removing the visible lesion does not by itself settle the question of further treatment or follow-up. Patients may seek care abroad for specialist pathology review, complex reconstruction or a combined skin cancer team assessment. Before choosing a programme, clarify the purpose of each operation, how the wound will be closed and when results will be discussed. Access to appropriate monitoring and drug treatment matters as well as the surgical technique; a cosmetic result cannot take priority over adequate cancer care.

An excision biopsy provides tissue for diagnosis. The report describes features such as thickness, ulceration and whether melanoma reaches the specimen edges. The team uses these findings and clinical examination to decide on a wider excision and any nodal assessment. Imaging is selected according to stage and findings, rather than automatically required for every patient. Review the whole plan within oncology.
A wide local excision removes additional tissue around the melanoma or biopsy scar. The wound may close directly or require a skin graft or flap. Sentinel lymph node biopsy is a separate staging procedure offered in selected cases. A tracer and sometimes dye identify the first draining nodes, which are removed for laboratory examination. It is not the same as removing all regional nodes.
A positive sentinel result leads to discussion of surveillance and additional treatment. It does not automatically require complete node clearance. Clinically involved nodes or other disease patterns may need a different surgical or drug treatment strategy. Ask how the result would change the proposed care.
Provide the pathology report, previous operation details, medicines and allergies. The team decides on local or general anaesthesia and gives instructions about fasting and medication changes. Discuss pregnancy or possible pregnancy if mapping uses a radioactive tracer. Arrange assistance after anaesthesia and clarify wound checks before travelling home.
Recovery depends on the site, size and reconstruction. Follow instructions for dressings, movement and protecting a graft or flap. Risks include bleeding, infection, wound breakdown, scarring and changes in sensation. Nodal surgery can cause fluid collections or lymphoedema; mapping dye can cause an allergic reaction. Increasing redness, fever, persistent bleeding or worsening swelling needs prompt assessment.
Pathology confirms excision margins and any nodal findings. Depending on stage, further surgery, immunotherapy or targeted medicines may be discussed. Regular review includes skin and lymph node assessment and scans where appropriate. Learn how to report new skin changes or lumps and follow advice about sun protection. Costs depend on pathology review, excision, reconstruction, mapping, anaesthesia and follow-up; later systemic treatment needs a separate estimate.
A wider excision reduces the chance of melanoma returning around the original site.
No. Suitability depends on tumour features and the expected value of staging.
No. Follow-up remains important because recurrence or a new melanoma can occur.
