Radiation therapy has a selective role in testicular cancer. It is mainly considered for pure seminoma, which is sensitive to radiation, rather than for nonseminomatous or mixed germ cell tumours. For suitable patients with limited spread to abdominal lymph nodes, external beam treatment may be an alternative to chemotherapy after removal of the affected testicle. The choice requires a review of the pathology report, tumour markers, imaging and longer term risks.
Radiation is no longer a routine recommendation for stage I seminoma. Surveillance after surgery is often preferred, provided the patient can attend the required follow-up visits. If radiation is proposed, the team explains the target area, preparation, possible nausea or bowel changes and measures to limit exposure of healthy organs. The exact germ cell tumour subtype matters. Pure seminoma is radiation sensitive; a mixed tumour containing nonseminoma is managed as nonseminoma. Radiation to abdominal lymph nodes is an option for selected stage IIA or IIB seminomas, after considering disease volume and other findings. More extensive disease generally needs systemic chemotherapy. An enlarged lymph node is not automatically proof of cancer, so uncertain findings may need reassessment before treatment is chosen.
After surgery for stage I seminoma, many patients can enter surveillance rather than receive immediate additional treatment. This is an organised programme of visits and tests, not an absence of care. Routine preventive radiation is generally avoided because many patients do not need it and late risks matter over a long lifetime. Where additional treatment is considered, the team should explain the alternatives and how the individual's circumstances affect that choice.
A planning CT scan defines the treatment area and nearby organs. Radiation commonly targets lymph nodes behind the abdominal organs rather than the removed testicle's position. Positioning and preparation should be reproducible. Discuss protection of the remaining testicle, baseline fertility and sperm banking before the course. Shielding can reduce scattered exposure but cannot guarantee fertility preservation. External beam radiation does not make the patient radioactive. The schedule and total dose depend on the specific indication and treatment plan.
Nausea, diarrhoea and fatigue may occur during abdominal radiation.
