Radiotherapy for cervical cancer can be the main treatment, part of treatment after surgery or a way to relieve symptoms in advanced disease. For many locally advanced cancers, the programme combines external beam radiation, chemotherapy and brachytherapy. These components have different roles: external treatment covers the planned pelvic area, while brachytherapy delivers radiation close to the cervical tumour. A recommendation depends on stage, lymph nodes, tumour characteristics and the patient's health.
Planning includes scans, an assessment of kidney function and discussion of previous treatment, medicines and any possibility of pregnancy. Pelvic radiation can affect fertility, ovarian function, the vagina, bowel and bladder, so these issues should be addressed before the course starts. The team explains preparation for each session, anaesthesia for applicator placement when needed, and how all appointments fit together. There is no single schedule suitable for every patient. Patients may travel for a centre able to coordinate external beam treatment and appropriate brachytherapy with systemic treatment, particularly when local access is limited; verify the complete programme before booking.

Early cervical cancer may be treated surgically or with radiation in selected circumstances. For many locally advanced tumours, chemoradiation with brachytherapy is a central treatment approach. After surgery, radiation is considered according to pathological risk factors, including margins and lymph nodes. An advanced or recurrent tumour may require a different programme, sometimes directed at bleeding or pain. Selected patients may also receive immunotherapy within an indicated combined regimen; this is not routine for everyone.
External beam radiation is planned from scans and delivered by a machine outside the body. Brachytherapy places a temporary radiation source in applicators positioned near the cervix; needles may be needed for some tumour shapes. Imaging helps plan this part of treatment as well. In definitive treatment of locally advanced disease, brachytherapy is an essential component and should not simply be exchanged for more external sessions. Ask how access to it is organised.
The team reviews staging, blood tests and kidney function before selecting chemotherapy. Instructions may include consistent bladder filling for external sessions. Applicator insertion can require anaesthesia and pain relief, which should be explained separately from the radiation itself. Bring records of previous pelvic radiation and current medicines. Pregnancy must be discussed before treatment. Fertility counselling should occur early, because pelvic radiation can damage the ovaries and uterus; preserving ovarian function does not guarantee the ability to carry a pregnancy.
Fatigue, diarrhoea, nausea and urinary irritation may occur. Symptoms can be more pronounced with combined treatment. Heavy bleeding, fever, inability to drink or severe pain need prompt contact with the team. Longer term changes can include early menopause, vaginal narrowing or dryness and bowel or bladder problems. Advice about sexual health, dilators when appropriate and menopausal symptoms should be individualised. Follow-up assesses recovery and disease control. Read more about brachytherapy and oncology.
No. Definitive chemoradiation is intended to treat the cancer without a routine subsequent operation. Other situations have different pathways.
External radiation does not. With temporary brachytherapy, precautions apply while the source is present; after its removal, it is no longer emitting radiation from the body.
No. Kidney function, other illnesses and tolerance affect the choice of medicine or whether concurrent chemotherapy can be given.
Contact the treatment team first. They should assess symptoms and organise any necessary adjustment while keeping the overall course coordinated.
