Whipple surgery, or pancreaticoduodenectomy, removes the head of the pancreas together with the duodenum, gallbladder and part of the bile duct. Some operations also remove part of the stomach. The surgeon then reconnects the remaining digestive organs. It may be considered for cancers of the pancreatic head, ampulla, lower bile duct or duodenum, and selected other conditions. Whether it is appropriate depends on the diagnosis, disease extent, relationship to blood vessels and fitness for major surgery.

A specialist team reviews scans, the likely diagnosis and overall health. For cancer, staging checks whether complete removal is feasible. Pancreatic cancer with distant metastases is generally not treated with a routine curative Whipple operation. Borderline resectable disease may require treatment first and reassessment. Tumours of the ampulla, bile duct and duodenum need plans suited to their own diagnosis.
Under general anaesthesia, the surgeon removes the pancreatic head and adjacent structures, then reconnects the remaining pancreas, bile duct and stomach or preserved stomach outlet to the intestine. Nearby lymph nodes are assessed in cancer surgery. Pylorus-preserving and classical operations differ in the stomach portion removed. The remaining pancreas stays in place; this differs from total pancreatectomy.
Open surgery is common. Laparoscopic or robotic surgery may suit selected patients in experienced teams. Sometimes the operation must change because disease extent or technical difficulties become clearer during surgery. Discuss that possibility beforehand.
Assessment covers heart and lung fitness, blood tests, nutrition, weight loss, diabetes and medicines. The team gives instructions on fasting and medication adjustments. Jaundice and any need for bile drainage are assessed individually.
After surgery, staff monitor pain, digestion, wounds and any drains. Activity and food intake increase gradually. A dietitian may suggest smaller meals or nutritional support. Risks include bleeding, infection, clots, leakage from reconstructed connections, pancreatic fistula and delayed stomach emptying. Diabetes or poor nutrient absorption may develop. Fever, worsening abdominal pain, persistent vomiting or breathlessness require prompt medical assessment.
