Comprehensive diagnosis of prostate cancer is a staged assessment rather than a single blood test. It may be recommended after a raised PSA result, an abnormal examination or other findings that need specialist review. PSA is produced by normal prostate cells as well as cancer cells, so a higher level does not by itself establish cancer. The urologist considers previous results, urinary symptoms, family history and general health before deciding which investigations are appropriate. MRI and, when needed, tissue sampling provide information that a PSA result cannot supply alone.
A consultation abroad may be useful when MRI and biopsy findings appear inconsistent, a pathology opinion needs review or the next diagnostic step is unclear. Share the scan images, PSA history and any biopsy report before travelling. Ask whether existing material can be reassessed, what additional testing would change the plan and how results will reach your usual doctor. Overseas assessment should not postpone necessary local care or be treated as a guarantee of a diagnosis within one visit.

PSA is a useful starting point, not a stand-alone cancer diagnosis. Benign prostate enlargement, inflammation and urinary infection can also raise it. Medicines and recent procedures may affect the result. The clinician reviews these factors, previous measurements and individual risk before recommending further assessment.
Multiparametric MRI examines the prostate and surrounding tissues. It helps identify suspicious areas and decide whether sampling is needed. A PI-RADS or Likert score describes imaging suspicion; it is not the same as a biopsy grade. Even an MRI without a suspicious finding requires interpretation alongside the other clinical information.
When indicated, a needle biopsy supplies tissue for microscopic examination. Samples may target an area seen on MRI or cover several parts of the gland. A transperineal biopsy passes through the skin between the scrotum and anus; a transrectal biopsy passes through the rectal wall. Ultrasound guides the procedure, and the team explains the anaesthetic and approach used.
The report describes whether cancer was found and, if so, its type and grade, including a Gleason score or grade group. Grade concerns the appearance of cells and likely behaviour; stage concerns the extent of disease. Additional staging scans are selected when needed, not automatically added to every diagnostic package. A negative biopsy does not exclude a missed cancer in every case: persistent concern may require follow-up or further assessment.
Provide previous MRI images, PSA measurements, pathology reports, your medication list and allergy information. Discuss recent infection and medicines affecting clotting. Do not stop prescribed treatment yourself. Before MRI, declare implants and difficulty with enclosed spaces. Follow the department’s preparation instructions for the planned biopsy and anaesthetic.
Prostate MRI commonly takes thirty to forty minutes. A transperineal biopsy may take twenty to forty minutes, but preparation and recovery extend the visit. Tissue results can take up to two weeks or longer. Confirm separate dates for each test and the results appointment rather than assuming everything finishes on the day.
Biopsy can cause bleeding, discomfort, infection or difficulty passing urine. Fever with chills, heavy bleeding or painful inability to urinate needs urgent medical assessment. Ask whom to contact after leaving the clinic.
Check charges for consultation, MRI, biopsy, anaesthesia, pathology review and indicated staging investigations. Agree how the final findings will be discussed and documented. Related information is available in the oncology section.
