EGFR, ALK, ROS1, PG-L1: best clinics abroad

We will pick a clinic for youFree, 3 options with prices →

EGFR, ALK, ROS1 and PD-L1 testing can help an oncologist assess treatment options, particularly in non-small cell lung cancer. These markers describe different tumour features: EGFR tests look for gene mutations, ALK and ROS1 testing evaluates rearrangements or gene fusions, while PD-L1 testing measures protein expression. Results add information to the pathology diagnosis; they do not replace staging or an individual treatment assessment.

Before ordering tests at another centre, ask whether material from your existing biopsy can be used and which analyses are already available. The receiving laboratory needs to confirm sample suitability and the exact scope of testing. Four named markers do not constitute a comprehensive modern molecular panel. Share the pathology report, previous biomarker results and treatment history so your oncologist can identify any gaps. Clarify specimen transfer requirements, the full quotation, expected reporting arrangements and who will discuss the findings with you before sending material or planning travel.

What these biomarkers measure

The tests answer different questions about tumour biology and may require different laboratory methods.

  • EGFR: analysis for mutations in the EGFR gene; the specific alteration matters when interpreting a result.
  • ALK and ROS1: assessment for gene rearrangements or fusions, which may identify a target for treatment.
  • PD-L1: measurement of protein expression using immunohistochemistry (IHC) on suitable tumour material, rather than a DNA mutation test.

Choosing the scope of testing

EGFR, ALK, ROS1 and PD-L1 are not a complete list of relevant lung cancer biomarkers. Your oncologist may recommend a broader molecular panel, including next-generation sequencing (NGS), alongside PD-L1 testing. Ask what the proposed panel covers and whether earlier results can prevent unnecessary duplication. Testing decisions depend on the cancer subtype, stage and clinical situation.

Using an existing biopsy

A new procedure is not automatically required. A pathology laboratory can assess whether stored biopsy or surgical material contains enough suitable tumour for the requested tests. Limited material or poor sample quality may prevent a reliable result. Before arranging transfer, obtain instructions on accepted blocks or slides, accompanying reports, packaging and return arrangements. Keep copies of the specimen identifiers and confirm which laboratory will receive the material.

What a liquid biopsy can and cannot show

A blood-based liquid biopsy can detect selected tumour-derived genetic alterations in appropriate circumstances. It does not replace tissue PD-L1 IHC. A negative blood result does not exclude every tumour driver; the team may recommend tissue testing if feasible. The suitability of either approach should be assessed before a sample is collected.

Reports, costs and clinical review

Request a quotation specifying the markers, methods, pathology review and any specimen transfer or consultation charges. Clarify when the laboratory expects to report after accepting the sample, and how any additional testing will be authorised. Your oncologist should review findings alongside other biomarkers, staging and prior treatment. A detected biomarker does not guarantee that a matching treatment will work.

Frequently asked questions

Does PD-L1 alone determine treatment?

No. PD-L1 contributes to an assessment that also considers genomic findings and the wider clinical picture. Discuss the combined results during an oncology consultation.

Are these tests the same as inherited cancer testing?

No. Tumour profiling and testing for inherited predisposition have different purposes. If a result raises concern about an inherited variant, separate assessment and confirmation may be needed.

How long will reporting take?

Ask the receiving laboratory for its estimate. Specimen retrieval, adequacy checks, analysis and additional confirmation can affect the schedule. Arrange the results consultation around the confirmed reporting plan.
