Waldenström macroglobulinemia is a usually slow-growing B-cell non-Hodgkin lymphoma involving lymphoplasmacytic cells in the bone marrow and a monoclonal immunoglobulin M (IgM) protein in the blood. It can cause anemia, fatigue, enlarged lymph nodes, nerve symptoms or problems linked to the protein. Some people have no symptoms when it is found. An abnormal IgM result alone does not establish the diagnosis: other IgM-related conditions must be considered.
A hematology team assesses marrow findings, blood tests, symptoms and organ function to decide whether monitoring or treatment is appropriate. Care is tailored to the effects of the disease, previous therapy and overall health. For a second opinion abroad, prepare marrow reports, available tissue materials, dated IgM and blood-count results, scan files and medication records. Ask how ongoing care will be coordinated locally. New vision disturbance, confusion, significant bleeding or breathlessness may signal hyperviscosity or another serious problem and require urgent local assessment rather than waiting for an overseas appointment.

Blood tests assess cell counts, the monoclonal protein and organ function. Bone marrow aspiration and biopsy help identify the abnormal cells. Specialized laboratory tests can include cell markers and selected MYD88 or CXCR4 testing. These results need interpretation together; a single mutation or protein measurement is not a substitute for the full assessment.
Imaging and additional investigations depend on symptoms and the clinical question. Risk assessment helps discuss the likely course, but it does not automatically determine when treatment should begin. A review within hematology and lymphoma care can clarify the diagnosis and which further tests are useful.
Selected patients without symptoms or concerning disease effects may have active monitoring rather than immediate therapy. This is planned care with clinical review and blood tests, not an instruction to ignore changes. Ask what findings would lead the team to reconsider treatment.
When treatment is needed, options can include antibody treatment, chemotherapy combinations or targeted medicines such as BTK inhibitors. Selection considers symptoms, blood counts, organ effects, molecular findings, other illnesses, previous treatment and preferences. The duration and monitoring requirements differ between approaches. Discuss adverse effects, interactions and infection precautions before starting; do not change prescribed medicines independently.
Excess IgM can make the blood abnormally viscous and impair circulation. Vision changes, headache, confusion, bleeding or breathing difficulty need prompt assessment. Symptomatic hyperviscosity may require urgent plasma exchange to remove circulating protein. This can relieve the immediate problem but does not replace treatment of the underlying lymphoma when that is needed.
Before travel, confirm medical stability, access to local urgent care and continuity of prescriptions. Send results in date order so the specialist can see trends. Ask whether original pathology material needs transfer, whether translation is necessary and who will communicate outstanding results. Request an itemised estimate and a written follow-up plan, including responsibility for blood tests and review after returning home.
No. The protein can occur in other conditions. Diagnosis requires specialist assessment, including the relevant marrow findings.
No. Active monitoring may be appropriate when there are no symptoms or other indications for treatment.
It removes circulating protein and can address hyperviscosity. The team separately decides the disease-directed treatment plan.
No. Seek urgent local medical assessment, especially if hyperviscosity is suspected. Do not wait for travel or a planned consultation.
