Liver cancer assessment first establishes what type of tumour is present. Hepatocellular carcinoma, or HCC, starts in liver cells; intrahepatic cholangiocarcinoma starts in bile ducts within the liver. Cancer that has spread to the liver from another organ is metastatic disease and follows the treatment pathway of its original cancer. These diagnoses should not be grouped into a single treatment package. The specialist team also assesses liver function, cirrhosis and the extent of disease.
For a consultation abroad, provide original CT or MRI images, reports, recent blood results and any pathology already available. Include hepatitis history, previous treatments and current medicines. Some HCC diagnoses can be established using specific imaging criteria in appropriate patients, while others need tissue sampling or further assessment. Ask which evidence supports your diagnosis, what additional investigations would change the plan and who will coordinate ongoing care. Treatment suitability, travel and costs require individual review rather than a promise based only on a scan report.

A liver lesion is not automatically primary liver cancer. The team distinguishes HCC, bile duct cancer, metastases and noncancerous findings. Previous cancer history, chronic liver disease and the pattern on imaging help guide this assessment. Cancer in a child needs a paediatric pathway, which differs from adult liver cancer care.
Symptoms such as unexplained weight loss, upper abdominal discomfort or jaundice can have several causes. New jaundice needs prompt local medical assessment rather than waiting for an overseas consultation. A referral for investigation does not itself confirm cancer.
In appropriately selected patients at risk of HCC, including some with cirrhosis or chronic hepatitis B, characteristic findings on multiphasic contrast CT or MRI may support diagnosis without biopsy. This approach does not apply to every liver mass. Indeterminate findings or a different suspected tumour may require further imaging, biopsy or specialist review.
Blood tests assess liver function and may include alpha-fetoprotein, or AFP. AFP alone cannot confirm or exclude HCC. Planning considers tumour size, number, location, blood-vessel involvement and spread, together with liver reserve and the person's overall condition. Ask which findings determine the proposed stage and treatment choices.
A multidisciplinary oncology team may consider resection, transplantation, ablation, treatments through the liver's blood vessels, radiotherapy or systemic medicines for HCC. Each has specific indications and limitations. Surgery depends on more than whether a lesion appears removable: enough functioning liver must remain. Transplant eligibility and access need separate assessment.
Bile duct cancers and liver metastases require different treatment decisions. Care may also involve gastroenterology to manage the underlying liver disease. Before travel, request a written opinion explaining the diagnosis, treatment goal, alternatives and monitoring. Clarify the need for pathology review, symptom support, included services and arrangements for care after returning home. Preserve copies of all imaging and treatment records.
No. It may have spread from another organ. Establishing the origin matters because it changes treatment.
No. Selected HCC cases can be diagnosed using defined imaging criteria. The team should explain why imaging is sufficient or why tissue is needed.
No. Tumour features, liver function, general health and programme eligibility influence the options. Other treatments may be more appropriate.
Send original scans, reports, pathology if obtained, recent laboratory results and treatment history. State your questions and ask how additional results will be reviewed.
