Medulloblastoma is a malignant brain tumour that usually develops in the cerebellum. It occurs in children and, less commonly, adults. Tumours are classified as CNS WHO grade 4, but this grade is not the same as the extent of spread. Modern assessment considers molecular features, disease in the brain and spine, tissue remaining after surgery, age and overall health. These factors help a specialist team plan treatment and discuss the individual outlook.
If you are considering care abroad, arrange review by a team experienced in medulloblastoma for the relevant age group. Send original brain and spine imaging, pathology and molecular reports, operation records and previous treatment details. Ask which findings still need clarification, what the proposed treatment aims to achieve and how care will continue at home. Significant neurological deterioration requires urgent local assessment; organising an overseas consultation should not delay necessary care.

Medulloblastoma can obstruct the normal flow of cerebrospinal fluid and cause hydrocephalus. Headache, vomiting, balance difficulties or visual changes can prompt investigation, but these symptoms have other causes. Marked drowsiness or worsening neurological symptoms need urgent assessment. Loss of consciousness requires emergency medical care.
MRI of the brain and spine helps assess the tumour and possible spread. Tissue obtained during surgery is reviewed by a neuropathologist. Cerebrospinal fluid testing can complement imaging; the team determines when sampling is safe and how to interpret it. MRI appearance alone does not establish the complete diagnosis.
Assessment includes molecular groups such as WNT-activated, SHH-activated and non-WNT/non-SHH disease, within which groups 3 and 4 are recognised. Additional molecular findings may refine the diagnosis and risk assessment. Results are interpreted alongside age, disease dissemination and postoperative imaging, rather than used in isolation to promise an outcome.
Surgery aims to obtain tissue and remove as much tumour as safely possible while protecting neurological function. Further treatment commonly includes chemotherapy and radiotherapy. Radiation may cover the brain and spine because the disease can spread through cerebrospinal fluid. The plan for very young children can differ substantially because of developmental risks.
Adults need a neuro-oncology team familiar with adult medulloblastoma; a childhood protocol cannot simply be assumed to apply unchanged. Treatment intensity and sequencing should reflect clinical and molecular findings. Clinical trials may be considered after checking eligibility, purpose and uncertainties; experimental treatment is not a guaranteed improvement.
When comparing oncology services, confirm joint review of imaging and pathology. Ask about translation and tissue-transfer requirements, the proposed timetable and what costs are included. Obtain a written plan for treatment, rehabilitation and follow-up near home. Monitoring can address recurrence and effects on hearing, hormones, growth, learning or memory, depending on age and treatment. Agree which clinicians will review results and how teams will communicate.
No. Grade describes tumour characteristics; separate assessment establishes whether and where disease has spread.
No. Tissue examination and molecular assessment provide additional information needed for classification and planning.
No. Age, biology, treatment tolerance and developmental considerations influence the plan.
Yes. The team should specify surveillance and assessment for possible long-term effects, with continuing support where needed.
