Tongue cancer can arise in the mobile front part of the tongue, which belongs to the oral cavity, or at the tongue base, which belongs to the oropharynx. These sites have different assessment and treatment considerations. Most cancers are squamous cell carcinomas, but biopsy establishes the diagnosis. A persistent ulcer, lump, pain, difficulty speaking or swallowing, or a neck lump needs assessment; symptoms alone do not prove cancer.
A head and neck team reviews the exact site, tissue findings, extent and effects on daily function. Surgery, radiotherapy and systemic medicines may be used in different combinations. HPV-related testing is particularly relevant to oropharyngeal tongue-base disease and should not be interpreted identically for every oral-tongue tumour. For a consultation abroad, prepare the biopsy report, original imaging, endoscopy findings and previous treatment details. Ask how the plan will address speech, swallowing, nutrition and dental care, and who will provide rehabilitation and follow-up at home.

Examination of the mouth, throat and neck helps establish where the lesion began. An endoscopic examination may be needed for areas not easily seen. Biopsy provides tissue for pathology, while imaging and selected lymph-node sampling assess local disease and possible spread. A photograph or scan alone cannot replace the relevant tissue diagnosis.
For tongue-base squamous cancer, assessment of HPV-associated disease, often using p16 testing, can affect staging and planning. This interpretation does not automatically apply to the mobile oral tongue. Ask the team to explain which site and pathological findings the report describes. A head and neck specialist assessment can clarify the required examinations.
Oral-tongue surgery may remove part of the tongue and, where appropriate, address lymph nodes in the neck. The extent depends on the tumour and safety considerations. Some patients need reconstruction. Radiotherapy or combined treatment may be recommended according to the disease and final pathology; removal of the entire tongue is not inevitable.
Tongue-base cancer may be managed with radiotherapy-based treatment, selected transoral surgery or another combined approach. Systemic medicines are selected according to disease extent, tissue characteristics, health and treatment goals. No surgical access method or device guarantees better cancer control or preserved function. An oncology discussion should explain alternatives and the consequences of each option.
Assessment by speech and swallowing specialists and a dietitian can help plan support before, during and after treatment. Discuss current swallowing difficulty, weight change and pain early. Feeding support may be needed in selected situations. Dental assessment is important when treatment could affect the teeth, jaw or saliva, particularly before head and neck radiotherapy.
Ask about rehabilitation, mouth care, dryness, taste changes and pain management. Reconstruction and rehabilitation aim to support function but cannot promise normal speech or swallowing for everyone. Breathing difficulty or significant ongoing bleeding requires urgent local medical help; do not wait for travel.
Send the exact pathology, relevant biomarker results, scan files and a list of medicines. Confirm whether tissue review is needed, translation requirements and an itemised estimate. Agree who will supervise nutrition, wound care and rehabilitation after returning home. Follow-up should assess recurrence, treatment effects and changing functional needs.
No. The sites belong to different anatomical regions and can require different pathways.
No. The required treatment depends on location, extent and the agreed clinical plan.
No. It informs assessment in the appropriate setting and does not justify changing prescribed therapy independently.
Discuss it early. Nutrition and functional support may be important throughout treatment and recovery.
