A dermatologist reviews the biopsy, tumour location, size, subtype and previous treatment. Mohs can be useful for cancers near the eyes, nose, ears or fingers, or those that recur or extend beyond visible borders. It is not needed for every skin cancer: standard excision and other options may suit lower risk lesions. Selected unusual tumours require specialist assessment. Bring the pathology report, previous treatment details and a medicine list, including blood thinners and supplements. Discuss allergies, implanted devices and relevant illnesses. Do not stop prescribed medicines independently. Confirm instructions about food, transport and wound care with the clinic, especially if reconstruction may require additional anaesthesia.
After numbing the area, the surgeon removes the tumour and a thin tissue layer. The specimen is mapped and examined for cancer at its margins. Further tissue is removed from mapped areas if necessary. Several stages may be needed, so the final wound size and length of the visit cannot be predicted precisely from the visible lesion.
Once the margins are clear, repair depends on the wound's depth, location and size. Options include natural healing, direct closure, a local skin flap or graft. Another specialist may perform reconstruction, sometimes on a different day. Ask who will manage dressings and any stitches after returning home.
Possible problems include bleeding, infection, discomfort, altered sensation, scarring and difficulty healing; a flap or graft can also have complications. Follow the individual plan for cleaning, dressings and activity. Seek prompt clinical advice for bleeding that does not settle, spreading redness, worsening pain or fever. Ongoing skin checks and sun protection help address recurrence and the risk of new cancers.
Both remove tissue, but Mohs combines staged removal with mapped microscopic margin examination during the procedure. Other techniques may assess margins differently.
Sometimes the visible lesion disappears after biopsy, while tumour remains underneath. The pathology result and examination still determine further treatment.
